Mg inhibits renal excretion of K in the cortical collecting ducts so hypomagnesemia would cause refractory hypokalemia unless Mg levels are corrected.
Hypomagnesemia is common with alcohol abuse & omeprazole use.

This is Aortic stenosis.
The stroke volume is significantly reduced & the left ventricular pressure is very high during & after the iso-volumetric contraction phase which indicates that the heart is trying to pump against a stenotic valve.

Dr Hammam has raised the bar a little to high,i can only dream of being half as selfless & dedicated to my patients as him”
But in his honour ill give it my all!
Rest in peace true hero!
So you feel “stressed out” studying for the USMLE?
I will have to learn how to live with myself without having had the honor of having this saint in my class.
This Palestinian doctor in Gaza reached a level of nobility the rest of us can only dream about.

My med school didnt hire any,i had to re-learn the art of learning all by myself & it has majorly impacted the way i study now.I think its one of the most important things a student can do to grow especially medical students as we are essentially students for life.
It’s been a while since i got done with step 1, still waiting for the high everyone talked about after passing.
But i must admit the sky wasn’t this blue & the grass this green before 🤭
Grateful ✨

When there’s a shunt (V/Q=0) in a portion of the lung leading to hypoxemia,100% oxygen will likely not correct the hypoxemia because the remaining alveoli that are being ventilated are maxed out & cannot extract more oxygen from the air.

Difference between cervical lymphadenopathy associated with EBV & lymphadenopathy associated with other causes of pharyngitis is that pts with EBV can also have generalised lymphadenopathy.
Wow that was a MOUTHFUL.

Sickle cell disease can cause macrocytic anemia because of 2 reasons.
1-Ongoing hemolysis leading to folate deficiency.
2-Rx with Hydroxyurea which increases MCV by releasing more reticulocytes.

I always always turn to Dr.Iqbals poetry when iam struggling.
His poetry is hors concours.
So for anyone struggling today remember its dark cause you’ve been planted, your time to blossom will come.
I will never forget the foul smelling diarrhoea caused by Giardiasis as opposed to the “normal beautiful” smelling diarrhoea association made by

TCAs are renally eliminated & since its a weak base the urine needs to be acidified with ammonium chloride to aid excretion.
BUT for TCA toxicity sodium bicarb is used to inhibit its blocking affect on sodium channels.

Low levels of Mg increases PTH release via feedback pathways increasing Calcium levels UNTIL the levels of Mg fall below 1mEq,now it can possibly cause hypocalcemia because Mg itself is important for normal PTH receptors functioning.
Today was a sign from the universe day which Iam really big on btw.
I was having a hard time studying & it was hour 5/12.What happens is this guy sitting across me absentmindedly pushes his pencil case towards me which says “Remember why you started”✨

I’ll be happy when I become a resident, it’ll make all this studying worth it
I’ll be happy when I am an attending… that will solve my burnout
It’ll be happy when I buy a house… or car… or…
This is called an arrival fallacy, and it’s a big problem for a lot of doctors

Ferritin, hepcidin & transferrin manage iron homeostasis but are also acute phase reactants working together to keep the iron scavenging bacteria at bay They make sure the bacteria have no iron to feed on in the blood.
Ferritin is the storage form sequestering iron in the tissues

Very true.Einstein was something else, part of what always makes him so great for me is not how smart he was but how but how he lauded curiosity & used his imagination to unprecedented extents. He taught me to believe in wacky ideas.
