One trick I have picked up (with variable mileage) is to phrase my barrier as a question for them.
e.g.
"Can you get onto the exam table?"
"Sure. Who are you grabbing to help me transfer?"
Best case: they solve the problem. Worst case: THEY have to be the one to decline, not me.

One time my partner was so sick she couldn't stand up, so I had to go in, pick up her store phone and call the boss to say she had to go home. He was like "you need to find a replacement" and I took great joy in telling him no, he did, it was his business therefore his problem.

I always used to use antiperspirants, and I routinely had painful lumps in my armpits, which were always itchy. Switched to using a deodorant instead and haven't had a single lump in the better part of a decade! Sweat is just the body cooling itself, I'm happy to let it happen.

I met my favourite four year old when she was not quite two, and at nap time I would sing her to sleep with lullabies in the native language of my home country. She's now a Big Girl and Too Old To Nap, but she still likes me to sing sometimes. It's the best part of our day.
