@DrBenLovell I think I tend to go with "something more worrying- something like a cancer". More of a warning shot than a euphemism. I don't like the vague, evil connotations of "sinister"

@dtbeartaylor @rupert_pearse @RoyalLondonHosp This isn't hospital admissions, this is intensive care. Small fraction of the total COVID+ patients in a hospital. Ppl tend to get this sick in 2nd week of illness, so if the virus is still spreading then likely to be many more this ill nxt week and where do they go if ICU full?

@ollieburtonmed This thread makes me sad. I might grump, but I actually love being Med Reg. I also think being confident in GIM is so important if you are doing a medically aligned specialty. Our patients are getting older and more complex, and we all need to be up to it!

@lauren_hookham I'm an endo reg. I find sodium sometimes extremely complicated! I ONLY ask (unless emergency of course) you have done the serums and the urines before you call. Then we can work it out together 🤞

@JonACB1994 Sick people are scary. If you're a reg and you don't respond to "I'm really worried, please come" or "I'm really unsure, please come" (or you can't identify that vibe, which isn't hard) then you shouldn't be the reg. (Or possibly I am a micromanaging megalomaniac but I hope not)

@DrHFRyan @hhjhammond Coming to hospital takes time out of people's lives, and costs money in travel. Some patients live very large distances away. Flexibility in the type of appointment is a good thing - patient choice is important here of course.

@techpriest I've had a consultant refuse to provide their name on the phone to me as reg before. (I mean they were on call, so it wasn't exactly a challenge). There is NO ROOM for this crap, and both on call cons and ed sup should hear about this

There is a bit of a systems issue with incidentalomas (as in there are loads of them) but that is a secondary care, not a primary care problem. Lots of units are developing pathways for endocrine nurse specialists, for instance.

@DrBenLovell Thank you for this thread Ben! I am coming to my last couple of months of "med reg" life. I will miss it. I get to use my experience to guide and support junior docs, to further the Ix of pts, explain to pts and their families, and much more. The best job. Full stop.

Helpful advice for all steroid dependent patients. Ensure you have AT LEAST 2 weeks worth of double-triple your usual steroid dose at home ready just in case!

@ruthlt82 I'm sorry you were shouted at, and that you were in the middle with this. This could be a great case for specialty M&M - "patient almost had to have procedure cancelled because cons in charge didn't sort anticoag plan in good time'. (I'm a hosp consultant)

@DiabeticDadUK @cahmn @sotonDSN >5 to drive... without further action. If 4-5 have a carb-containing snack before you set off, but you can drive. If less than 4, treat to target of >5, then WAIT 45 mins at least before driving. Important that CGM alarms are NOT considered a substitute for hypo awareness

@BenKentish I am sorry your mum had to wait so long Ben. The trouble is that if scan really IS urgent, someone needs to see and act on the results urgently. GP cannot. This is remit of acute hospital services. The use of ambulatory care helps a bit with this, but often not 24/7 (1/2)

If ICUs are full of COVID - even if not shipping out - that has a massive implication for the hospital. Postponing elective ops e.g. for cancer etc. People will suffer.

@DrBenLovell Ben I think it's a shame when we have so much changing in diabetes. Interstitial (flash and CGM) monitoring, insulin pumps, new oral and injectable treatments in the clinic ... but in the wards we use an acronym for test strips that stopped being made in what, the 90s? 🤷‍♀️

@Parody_RCGP This is why it's good to address the letter to the patient. Then the (probably not bold!) text is directed at them - "we really want to see you and support you..."etc. GP knows we have tried (we phone our DNAs often) but unable to provide care, and the onus is on the patient

@DrLindaDykes I have a little list - adrenal insufficiency *** (often so obvious in retrospect but many patients wait a really long time for diagnosis) - aortic dissection (can present in such strange ways) - gastric volvulus (seen it as "cardiac chest pain") - necrotising fasciitis

@Lucie_911 Have you used the HMRC Online system? I recommend doing this every year if your salary/employer changes. I don't think payroll can do this, but if you put your earnings for the tax year on the portal, HMRC will calculate your correct code for the rest of the year
