AA debate. If you want to ‘play’ anaesthesia & be a regionalist. Go to med school. Rack up the debt. Do the FY stuff. Pass the FRCA. Learn the anatomy. Do the rotational training. There is no short cut. There maybe a role for the AA to support the anaesthetist. But not to be one.

Today 350 anaesthetists, trained purely in anaesthesia for the last 3 years, were turned down for the next stage of anaesthetics training in the UK. This is despite @RCoANews stating there is a "current shortage of 1400 anaesthetists in the UK"...🧵

I’m a doctor. I completed 6 years at medical school, followed by 2 foundation years, working hard jumping hoops and even then wasn’t able to get an anaesthetic job. Tell me more about how people without medical degrees feel hard done by without having had to do the same graft.

I received 3 complements this AM. 1. surgeon saying my anaesthesia was good. 2. ODP saying I blocked like a ninja. 3. nurse saying my hair looked nice. I wonder if the NHS would fare better if we ‘bigged’ each other up more often, rather than what feels like a race to the bottom.

I will stand firm on my views. You can teach anyone to stick in a needle. But it takes time to create a regionalist. Lists such as awake hands are essential to learn the building block skills. If we farm them out to AAs in the name of service, our speciality has no future.

My first awake upper limb list in 15 months 😬. Task = find the ICBN. 1st pt. Dodgy tourniquet pain. Disappointed (but pt did invite me round for a cider 😂). 2nd pt.💥 Great block, comfy happy pt. Moral = get back on the 🐎 & keep trying! Thanks @LloydTurbitt for the teaching!

If we have the training capacity, why not just increase training numbers? If we don’t have capacity, we can’t train AAs either. We should also support SAS career paths.

Today we had far too much fun. I introduced ‘the stick’ of regional Anaesthesia 😂 Set up a tea trolley. Took a complete novice from never picking up an USS to a beautiful SIFI & LCNT in under 3 hrs. Excellence in RA training is possible! Ta to the pts for being good sports!

This is common. We are failing trainees at the most basic level. Many feel they need to do advanced training just to get any level of skill whatsoever. It’s a priority for our speciality to invest in grass roots training for its future.

Nothing spoils my day quite like a surgeon asking for a gastric tube in a patient head up under the drapes.

@Kirann77 Well that’s the point. We are losing doctors in droves. Other countries look so much more appealing. If we don’t start retaining doctors now, one day there will be very few left!

RA is humbling. Boss says ‘you’re in charge’. I opt for ampres spinal. Surgeon unconvinced. I say it’s best for the pt. Pt agrees. Boss asks how are your spinals? ‘Good’ I reply. CAN I GET THE BLOODY THING IN? I try EVERYTHING. Street cred in tatters. Back in my box. #SadFace

@RobJimFleming @Nabeela_Surgeon It’s everywhere Rob. Great doctors are suffering. I’m personally fed up of hearing ‘it’s on our radar’, ‘change is happening slowly’. But I don’t see it. And it doesn’t help people like me who are constantly walking through treacle with no allies locally & a lack of belonging.

⁦ @LloydTurbitt ⁩ makes excellent points in contrasting axillary vs infraclavicular for NOVICES #RAUK21 . It is less daunting, and works well if you’re always at level of conjoint tendon & use Lloyd’s “3-pass” approach - 1/ below artery (RN) 2/ above artery (MN+UN) 3/ MCN

Really disappointed in this differential attainment article from @RCoANews . Can disabled people not be anaesthetists? Sure seems so. It’s becoming the biggest regret of my career. Not representing us AGAIN. Remember you can’t be what you can’t see. We need to do better.

🔓The first results from @RCoANews @NAPs_RCoA are now available! ⬆️Age and BMI has increased since NAP5 in 2013. ⬆️TIVA use has more than trebled. ⬆️pEEG use has increased more than 6-fold. No increase in regional anaesthesia or spinals. 🔗

So it’s DIT change over. Just met one of the CTs. ‘You must be Becki. I’ve heard a lot about you. You like the block’. I replied ‘like is a loose term. Not sure that’s how I’d describe it’ #LifeTakeover Is this infamy? Am I anaesthetic legend? 😂Made me smile for two minutes.
