After a long wait, our longitudinal long COVID study is finally published in
@NatureComms
: . We find a skeletal muscle alterations in patients with
#longcovid
, which worsen with exercise. 1/n
@BraedenCharlton
Skeletal muscle mitochondrial function and metabolism were altered in patients with
#longcovid
, and this worsened after induction of post-exertional malaise (PEM). Intense exercise is not good for patients with long COVID! 4/n
@BraedenCharlton
We obviously have NOT sit still in the time that this publication has been reviewed. We hope that in the near future we can contribute more knowledge on how exercise links to the disease, and what can be done to reduce PEM. Stay tuned! 9/9
We can finally announce that we have been awarded almost €500.000 for our work on understanding post-exertional malaise in Long COVID. We are thrilled that
@patientled
is going to support us!!
Big news!
We are *thrilled* to announce the $4.8 million in research awards for
#LongCovid
& associated conditions from our Patient-Led Biomedical Fund today!
These were decided by a panel of 15 patient-experts & we're funding 9 exceptional projects!
1/
@BraedenCharlton
To reduce the skeletal muscle abnormalities (reduction in mitochondrial function, muscle damage and more microclots), we can only recommend to avoid strenuous exercise for now. Every patient likely has a different threshold for induction of post-exertional malaise. 8/n
@BraedenCharlton
Blocking of capillaries by amyloid-containing deposits (or microclots) could reduced exercise capacity. We found these deposits in muscle tissue, but surprisingly not inside capillaries. They were located in between fibers, were higher in patients, and went up after exercise 5/n
Our BSc students are presenting their results about
#PEM
in
#longcovid
and
#mecfs
. Lots of work, and giving us another 🧩 pieces to understanding PEM. We now check their data, and will add more results before writing this up for a follow-up publication. Great work guys!
@VU_FGB
I have always dreamt about publishing a wildlife photo in
@NatGeo
, but a full article about our work on
#longcovid
, I am extremely honored! Thanks
@rachel_fairbank
!
With
@BraedenCharlton
, and clinical colleagues (Brent Appelman and Michele van Vugt), we confirmed the lower exercise capacity in patients with
#longcovid
, which can partly be explained by changes in skeletal muscle structure and function 2/n
@BraedenCharlton
This study contributed to our understanding of muscle pain, fatigue and weakness in patients, and provides a biological substrate for the disease. It doesn’t directly point to the cause of the disease, but highlights that exercise can worsen symptoms in patients 7/n
@BraedenCharlton
We next wanted to find out if skeletal muscle abnormalities contribute to the feeling of muscle pain, and fatigue one day after an exercise test on a bike. We found that in some
#longcovid
patients there was extensive muscle damage, but also signs of repair 3/n
@amsterdamumc
@VUamsterdam
@VU_FGB

@ZonMw
Last but not least, we really want to thank all patients who participated, who stepped on that bike knowing that they will feel worse the next days/weeks. All the patient contacts online, via email, in person, everyone helped to put the 🧩 together! Together we make a difference
@BraedenCharlton
We tested whether viral persistence contribute, and screened muscle biopsies for the N-protein. We saw equal amounts in patients and healthy controls (who recovered after COVID). No viral particles were inside cells, so we assume remnants and not active viral particles 6/n

@NatureComms
As an exercise physiologist, I found it “strange” to hear from patients that something goes wrong with exercise…. And then i received a phone call from a doctor with the same question… 🤩
@AlanCarson15
Dear Alan, please bear with me for 2 weeks, when we publish our results of a bed rest study in Cell Reports Medicine. I am also excited to see this work replicated elsewhere, but this work is in line with previous studies on similar topics (even one dating back from 1980s🙃)
@EleanorDavin
@NatureComms
We are now seeing if something similar happens in patients with ME, and hope to include muscle biopsies from patients with severe ME as well in the near future
@ZonMw
@VU_FGB
@AlanCarson15
Also, it’s important to remember that people undergoing bed rest get better when they start exercising again, but this is not the case in long COVID patients. They get worse…
Vanavond wordt in het programma van

We are super excited that this post from

about our work, currently presented at an international conference in the US, gets a lot of attention. We read along, but as we have a delay with the publication, we hope to explain more in the near future. Stay tuned!
Wonderful to see the
@RobWust
team at the

symposium. They found amyloid deposits, identified by ThT in the muscle. Perhaps due to movement of the amyloid deposits via the damaged vasculature?
This makes me silent. As a fellow-Dutchie, I am extremely honored that I have been invited to speak about post-exertional malaise in long COVID and ME/CFS at this worldwide online conference. You can still sign up (free, and online)!

Hi Eric, that there is more going on than just mitochondrial dysfunction in LC, as also ex-professional athletes/bodybuilders can suffer from long COVID & PEM, despite having a relatively ok exercise capacity. The pathology of reduced exercise capacity is different from PEM

We just published a letter about the critical need for a clear definition and detailed pathophysiology of Post COVID-19 condition (

). We feel that including hospitalized patients and those with comorbidities blur the real LC pathophysiology.
@riendebock

Hi Nikki, this is a great question. We are aware that also mention/cognitive exertion can cause PEM, and colleagues in the Netherlands are actually studying this right now. Does “crashing” after mental also involve muscle pain?

Likely not. Because it is likely that the mitos are not the core problem, but responding to a unknown factor X upon exercise. You might be able to reduce PEM intensity, but I think that the current drp1 drugs are not “for human consumption “

We are obviously aware of this study, but we added more technology and measurements than published before. Also me/cfs patients likely have the disease much longer than our patients who had LC for only max 2 yrs, and did not suffer from deconditioning and comorbidities
Stay tuned!

The skeletal muscle prevented further nutrient accumulation by making its membrane less susceptible to glucose uptake. Is a decrease in Insulin sensitivity maybe a positive adaptation of the body to prevent further detrimental effects of a local nutrient overload?

@NatureComms
Hi, most of our patients got long COVID before the vaccinations. Also: we compared our data with pre-pandemic data, and there’s no difference. Only difference is that pre-pandemic didn’t have any N-protein….
Even ter verduidelijking: we denken erover na….
Surprisingly, the accumulation of toxic lipids coincided with a shift from fat to glucose oxidation and changes in mitochondrial structure and function. This buildup of toxic lipids did not worsen insulin sensitivity, changing our understanding of the development of diabetes
@cstroeckw
@AnilvanderZee
@NMCB_studie
But…. Our funders are now more and more critical in studying PEM in patients because it (obviously) negatively affects disease symptoms. So whether we can actually continue obtaining more samples remains to be seen
@FvRhijn
