Today, we mourn the passing of Hein J. J. Wellens, MD, PhD, a founding pioneer of clinical cardiac EP. His contributions to the field forever changed how the global EP community diagnoses and treats patients with heart rhythm disorders. Dr. Wellens, you will be missed.
The more atrial fibrosis, the greater chance for AF recurrence after catheter or surgical AF ablation. Dr. Thomas also discusses importance of quantifying LA function with AF therapies.

Cephalic vein remains preferred approach. If you pass the suture sleeve into the vein (and place silk tie around the sleeve + vein) there is no bleeding at all and bet this technique would also win out for bleeding events.
Metanalysis: as compared to subclavian puncture, lead insertion of CIEDs via cephalic vein cutdown yields lower risk of pneumothorax (0.19% vs 1.30%, OR=0.21) and lead failure (0.50% vs 1.92%, OR=0.25) without difference in the incidence of bleeding events

Hard to compare results of EAST-AFNET4 to older trials and this current study looks at EARLY rhythm control (diagnosed within 1 year before enrollment; median time to dx 36 days). Compared to older studies, findings suggest we should treat AF early with rhythm control!
