When I started residency, I tried several ways to save articles for later reference for learning, and also to use for academic writing. I tried folders on my computer, Google Drive, and Zotero. Then I found Papers by

The best advice I’ve ever gotten from an attending was to prepare for every day and every case as if he wasn’t going to be there. My education has improved exponentially since trying to implement that ideal

I had a couple of interns/juniors ask how I prepare for cases, so I shared my thoughts in a thread:
1. If you’re starting out, the most important thing is to identify what you’re treating and what the options are.
Ex) mid-shaft femur fracture can be treated with antegrade or
Is it worth it to run home to see your wife and little dudes awake during a painful OR delay on a busy call weekend? Absolutely is.

if you have a family, live close during residency. Game changer.

Doesn’t matter what your specialty is, if you’re closing lacs in babies or toddlers please use absorbable sutures, so residents on the hand surgery service don’t have to take them out two weeks later 🤪
Starting to use ChatGPT to help with academic writing. It's definitely not safe for writing papers, but it can help you go from a blank screen to an outline and give you a springboard to go from. Don't trust its citations thought!

At a dinner/chapter review with an attending who buys for the on-service resident and fellows. I order an extra meal on a separate check for my wife. He picks up that check, tells the waitress to add it on, and refuses to let me pay.
I want to be like him when I grow up.

I’m not kidding, our institution sent all employees a commemorative thank you coin in the mail. I’ve never seen the OR staff more universally angry about something.
New thoughts! Resident autonomy has declined dramatically over the past two decades, with some potentially significant consequences. Curious what senior experienced staff think.

I came home to discover that my mom had taken 20 years of carefully organized series of books and sorted them BY COLOR. Now I can’t find anything 😂🤦🏻

Journals often require 2 year minimum follow up for any clinical study…but for short-term outcomes studies is 1 year enough? Maybe for some things it should be…

I spent a lot of time at various points in my training asking myself the same question. Theee state schools before being lucky enough to be at Cleveland Clinic for fellowship. I’d say two things:
1. Don’t over value the institutional names. It is definitely helpful to be at a
Every so often we lose a patient, usually hip related. Reminds me forcefully that we aren’t “just carpenters.” We have to help people choose between dying slowly in bed or trying to get back to some semblance of normal but risking a surgery. Challenging when the worst happens.
Any tips for finding the interval between biceps and brachialis on anterolateral approach for humeral shaft fractures? Found it challenging the last couple of times. Other than to not fix humeral shaft fractures 🙃.

Fellowship: you love shoulders? Let’s talk about shoulders, operate on shoulders, do labs and lots of simple and complex shoulder cases, and eat/drink/breathe shoulders all day every day 😍
Easy to see why people say this is the best year of your career!

Absolutely blown away by the response! My wife has never really had a plan to make and sell these but we’re working on it and hopefully will have it available in a few weeks.

Crazy and awesome to see this in press. Saw a lot of

A thread on suggestions for interns/juniors getting through a stretch of nights or night “float” shifts:
1. Make your health a priority. The job will get done, but you need to stay healthy otherwise you won’t be able to do the job well or stay motivated and happy.
Reminder for rotators across all specialties as you get through subIs: it’s scary, but ALWAYS ask if someone is comfortable or would be able to write you a STRONG letter of recommendation. It is possible for someone to just write you a bad letter - ask me how I know.

It’s nice to get some good news. Very excited and humbled to see my first project in shoulder surgery in print! Grateful for amazing mentors like Drs. Lederman and Gobezie and their partners

People in other specialties aren’t morons. They’re just not trained in your specialty. Please get over yourself.
Sincerely,

It’s honestly incredible how much of an impact small acts of kindness and smiles have. I wonder how it would affect patient outcomes if we were just happier and kinder in every encounter, regardless of whatever nonsense was making the day harder.
Virtual conferences have had a huge impact on dodging tough questions. “I’m sorry my mic wasn’t working” has replaced “I’m not sure what you’re asking” 😂

Convo with buddy in finance:
Me: “sorry I’ve been super busy, someone’s out with a health issue so we’re covering extra shifts”
Friend: “I hope they’re paying you for the extra work”
Me: “...thought didn’t even occur to me”
Different world.

This isn’t comprehensive at all, but hopefully somewhat helpful as you transition from “OMG broken bone” to “how do I fix this tomorrow when I go in”. Ask and learn from everyone so you can figure out how you learn best.

8. After every case, try and write down what you picked up or learned that was new, as much as you can remember. Periodically go back and organize those notes as you learn how you will do things, what you liked and didn’t like from different attendings.
@RyanMarino
I used to think like this until I rotated in the addiction clinic as a medical student and a patient told a story of how they’d come home from 3rd grade and their parents would be passed out from a heroin high in the couch. If only they’d chosen different parents.
As I approach the end of fellowship, there’s one question that no one has ever been able to answer…when (and who) can you start calling people by their first name? 😂

The privilege I have in my life is due largely to my dad, a hospitalist who taught me (and still does for my kids) to give whatever it takes for my family and kids. Hope I can do the same for my own. Happy Father’s Day!
