🧵 Diagnostic cardiac caths... We do quite a bit of them. Most are older kids vsds with severe pulmonary hypertension for assessing pulm vasc bed & its reactivity to vasodilators But some really interesting & challenging ones r for delineating MAPCAs in ToF with pulm atresia. +

Quite an adventure last night doing Rescue PCI for 66/M/2hrs post tenecteplase failure(outside hosp)+1 V.Tach x DC shock,timely PCI with 4x 20 mm stent.Multiple inotropes got tapered off immediately post stenting.All izz well⚡️💯 #CardioTwitter #MedTwitter #cathlab #adventure

Yess..these are canonball mets..the primary was liver on PET scan.Hepatocellular carcinoma.patient was found to b equivocal positive in HCV immunoassay.HCV RNA quantification is pending.HCV has been recently associated with increased CV mortality @doctorboletoh @KailashKolhe

Emergency call- post AWMI patient(1 month back) class IV symptoms-EF 30%. kept On Bipap by RMO thinking CCF. Bt Was Dressler's in Tamponade- 550 ml bloody fluid tapped.RMO hd written a diagnosis- CCF-Pulmonary edema+ cardiac tamponade can u guess what is wrong with his diagnosis?
