here is the link to our institution's experience with pediatric heart transplantation. I hope it gives a good idea of the current scene here. This is a repost from a month ago; my original tweet got deleted inadvertently.

Not acceptable:
That u will be deciding ranks of the 67 students who scored 720/720 on non academic points.
Rather, u should have anticipated this situation and already put in 12th marks /10th std marks etc as deciding points BEFOREHAND.
Yet another farewell party...
Sis goes off to a super setup in Europe.
There is no way to stop this.
We can't match the pay/benefits of the West
We train the ICU nurses( she wasn't ICU though) & 18mo later ...✈️
They absolutely deserve better than what they are offered here

Every year this young lady without fail comes to the hosp and celebrates our birthdays with us.
She is enjoying her second life to the fullest.
Before her heart transplant, she was bed-bound...no energy for anything.
Now she attends dance classes!
People want good doctors for themselves and their families...
... But
Will happily ignore the crisis going on right now in Indian medical training system.
When will u speak up folks?

I am on
@Twitter
.
I am a pediatric cardiologist.
There are very few of us on twitter. Most are off twitter.
Me being on twitter doesn't make me the best of them ( nor the worst😊).
Twitter presence isn't a factor in deciding how good or bad a specialist is.
In any field.
Had the chance to hold a pediatric cardiac camp in Chandrapur recently .
Saw so much pathology. We have come a long way in India but still a long way to go ahead as far as pediatric cardiac care.
This post for LOW IQ people only:
I am Pediatric Cardiologist.
I dance.
I wear what I like.
I eat/drink what I like.
And yet every working day I take care of my patients in the best way I can.

from my archives. This little pt had HOCM; was on beta blockers; not compliant; came with chest pain. Initial echo showed his usual severe HOCM. Within the next 48hrs hours, his echo changed to this. no fever. high crp. unrelenting chest pain. Tapped him: thick purulent fluid
I like
@SadhguruJV
. I like how he explains things about Hinduism.
I have never heard him be anti modern medicine .
He is pro Yoga and pro Meditation. But so am I.
And I am also a modern medicine practitioner.

Specialities like ours (pediatric cardiology and ped card surgery and ped card Anesthesia and ICU) are not popular in India now.
Compensation doesn't match the hard work put in.
Passion doesn't compensate for EMIs and school fees.
So why not opt for more paying subspecialties💔

Street dog attacks are often unprovoked.
They attack in packs.
They attack alone too
They attack kids and adults too sometimes.
They are a menace.

The tanned appearance of the back of the neck of your overweight/ obese teen isn't that.
It is acanthosis nigricans. Abnormal darkening and thickening of skin there. U may find it in their armpits too
It isn't a good thing
It likely means that the teen has insulin resistance🧵

Unsolicited advice to all my junior colleagues
No job is below our " level".
Do what it takes to get the pt to be better, with what is in yr control.
Be smart. Find a way out to work well with nurses, wardboys, ayahs and seniors ofcourse.
Yes it is tough.
Medicine is not only
Stop moral policing of doctors .
What they do outside their place of work is their choice.
As much as it is the choice of any other law abiding citizen of India.
My Saturday is made😌
Got a compliment from 2 lovely older ladies from my bldg for my saree as I walked out.
Just the other day , one of them told me proudly, " I am 84. I am healthy coz I walk daily"
I wished her many many more morning walks....
My 2 cents.
Dear resident in distress:
U have to strengthen yourself.
Coz even if the system is at fault for your distress, it may not change in time to be of help to you
So work on yourself
Look at what the psychiatrist (seek them) is saying: same thing.

I don't appreciate women's day messages that say... The world revolves because of u. ...u are a great multitasker. ...etc etc

people are giving advice on what to do if stray dog comes after you or to prevent being chased altogether....
"Stay calm"
"Don't carry a stick
"Don't carry pepper spray"
"Stray dog will not do anything if you are kind"
" This has never happened to me"
Laughable really.
I knew being a parent to a teen was tough... But I am having a really tough time these days.

The Indian govt has great schemes for kids esp in rural areas.
No reason for children to remain uncorrected
Wrong advice given by extended family/ quacks is 1 reason
If a person can't afford a private hospital fees, doesn't have insurance (no foresight); doesn't qualify for charity help there, there is always the nearby government hospital to go to.

This infant had heart surgery the day prior.
Now they are ready to feed and feed and feed some more.
Most kids after repair of large VSDs develop a ravenous appetite and in the 1 month post repair gain 600-800g weight.

A baby is born w Ebstein and cardiomyopathy. Gets an open ❤️surgery for it.
Then gets a heart transplant some years later .
She is now a preteen. This ❤️fails so she then gets fitted with a total artificial heart and 2months later gets another new heart from a donor.
Amazing

The inspiration was lunch time banter recently, with work colleagues...chiding each other to cycle on a Sunday AM rather than sleeping in late.
I did it! 😌
