The answer is AVNRT with aberrancy. Fusion beats are virtually pathognomonic of VT but this seems to be an exception to the rule: SVT with aberrancy and a well-timed PVC from the ipsilateral ventricle (eg RBBB aberrancy and PVC from right ventricle), resulting in a narrower beat
