@jfitzgeraldMD Nah OB was different. My surgery rotation as a Med student and my intern year in general surgery was predominantly women. High stakes but still felt like we’re in this together. OB rotation was like lord of the flies. Very clique-y and not remotely inclusive.

@CMerandi All due respect, but you’re misrepresenting what ERAS is. Goal is to minimize opioids and use a multimodal approach to reduce postoperative pain and quicken recovery after surgery. People aren’t left to suffer. Please keep your uninformed (and biased) opinions to yourself.

@Vancbromycin I had one of those when I was an intern. Nice as could be, proactive and hard working. It was amazing. I wrote him a glowing review and told clerkship director that he should be able to go and do and whatever he wants. He does pediatric cardiac surgery now. Seems appropriate

@kjdelay1 This won’t get better as long as physicians continue to work for large organizations. We’ll always be cogs in the wheel. Widgets. Single specialty practices fix a lot of this. But it’s riskier and a lot of folks (particularly our generation) are afraid of that risk.

@arghavan_salles The “good” men a lot of women are looking for do exist. They just aren’t interested in dating a certain brand of American women. They date/marry conservative Americans or people from other countries. There’s a particular female culture here that is just very unappealing

@arghavan_salles I can think of at least two people who only got into our training program based on the DEI paradigm. And they had multiple patient complaints, instances where patients were harmed and were still graduated and released into the world.

There is simply way more work for me to do compared to in an over saturated city-based market. The issue is my generation doesn’t want to live out there or commute

@AjKavanaugh I’m gonna say it over and over. Working for large health systems is why people burn out. Loss of autonomy is the issue. We traded it away as the administrative burdens of healthcare rose while health systems were happy to take that on in exchange for control of our labor.

@olsonplanner Definitely. My wife and I are both docs. We finished training with 570k in loans. I’m four years we’re down to < 100k in loans, have two rental properties (homes we previously lived in), want for nothing and have deeply satisfying jobs with great work life balance.

@alextatem I’ve always thought we don’t really understand post prostatectomy ED. I’ve had patients with terrible disease, no nerve sparing, and they’re getting erections 3 months postop. And folks with beautiful nerve spares, excellent preop SHIM, and no erections afterwards. Crapshoot IMO

@residencyreview Sounds like any surgery program. I’d much rather have a surgeon that trained in that sort of system than one who was coddled. The former will almost always be the better surgeon. Surgery is not for the faint of heart. Not everyone can do it

I always offer a referral for more complex cases but most of my patients don’t want to drive to the city. If I’m comfortable doing the case and they prefer I do it, then I’ll do it.

@SPuro88 I used to do the same thing in reverse as a resident. Wife was internal medicine. I would work up these complex EKG changes or electrolyte abnormalities overnight. Present to my chiefs the next day and they would be like “what the fuck did you do?”

@PoleDoctress The unintended consequence of inclusion at the expense of quality is distrust. When you lower standards to allow people in that look a certain way, people will question everyone that looks like that. DEI dilutes the achievements of the URMs that actually did the work.

This happened because of increasingly burdensome regulation and a move towards shift-work to improve work life balance.

@DrPayItBack A busy private practice doctor (at least for urology) generates around $500-600 an hour in collections. Half of that goes to overhead in a lean practice. And then of course the more middle men between you and your cut, the less your share…

@jfitzgeraldMD I have a handout I give em. Tell them the front side is for their doctors. Lists uti symptoms and then quotes the guidelines regarding not treating asymptomatic bacteriuria. Please call urology clinic if any issues. They still get antibiotics…

@arghavan_salles We also had residents who were from historically underrepresented groups that were phenomenal. And they would have gotten in regardless of their race/gender/ideology, and based purely on merit. All DEI does is tarnish those people’s accomplishments.

@DrPayItBack Unless it’s actually your business and you have a head for the entrepreneurial side of medicine, it’s not a great risk/reward scenario. A rubber stamp medical director makes around 25-50k per year at one of these operations. Not worth the liability.

@RianKMD Reducing work hours may lead to either lower quality training or require longer time in training. There is value in those long hours. Recent paper in NEJM showed LESS complications with residents working 24hr shifts vs 16hr. Presumably due to less handoffs and more continuity.
