I don’t blame the ortho surgeons. I blame the people who agree that gynecologic surgery should pay less than ortho surgery. Because we all know that’s how this happens. Over balance sheets with CFOs finding $$ to hire the extra tech+nurse to make sure their cash cows are happy.

@kevinault @janevandis Unequivocally. A ruptured ectopic pregnancy requires surgical intervention. I’m no hero, but now more than ever fearlessness to do the right thing is required. Knowledge of the law to the letter requires it. In this case it doesn’t even require risking breaking the law.

I should clarify that I used exception incorrectly here. Many states require an AFFIRMATIVE DEFENSE. That is, you can stand accused of the crime for the abortion, but evidence of emergency negates this.

@AngelaM_SLP @dremilyportermd Elective sterilization is absolutely acceptable even in Texas. The state payors (Medicaid) require a form be signed at least 30 days prior to elective sterilizations for the *indication of sterilization* but the signature is often good for 6 months.

@MrsPA_C Yikes, it would be helpful to know if the employer has a "default to positive enrollment" if no action is taken. Last year, I did the process but the enrollment system didn't log the action even though I made it to the confirmation page. I got re-enrolled by default.

There are real consequences to mismanaging endometriosis. I met a patient once whose was referred to a gynecologist with known advanced endometriosis. The patient underwent hysterectomy BSO as "menopause will cure it." 1/
