My sense is some men choose surgery over RT due to concerns over secondary malignancy. If you discuss secondary malignancies of RT (as I do), be fair and explain it appears to be an absolute risk of 0.5-1% at 25 years (many docs don't give risk estimate)

Advice from a random joker....re: AUA talks: - obsess about every detail on each slide (format, spelling, symmetry, font, color, brevity) - practice until you can give talk without your slides - eye contact with audience - don't be the schmuck who goes over time limit #AUA19

In my utopian world, every prostate cancer doc and patient with early stage PrCa understands details of this landmark trial, counseling fairly reflects the data, including 'active monitoring' from 1999-2009 is very different than active surveillance today

I continue to struggle to comprehend Rad Oncs and Urologists who universally recommend their treatment for men with PrCa who clearly have both as options. Physician-centered care is anathema to proper discussion and delivery of health care.

I’ve lived in Israel and visited/worked in West Bank-Gaza about a dozen times, including with wife and kids... If you think one side is categorically “right” and the other “wrong”: you are reprehensibly misguided. Advice: be quiet, pause, read, listen, and learn.

Our work showing MRI underestimates PrCa tumor volume, often dramatically. Volume significantly underestimated on T2, ADC, and DCE compared with pathology by 55% (mean), 59%, and 18%, respectively. DCE best. Similar to work by NYU @tanejauro and UCLA

Blow up the Gleason system, it fails many men. Saw 3 guys this week with a single core of very low volume pattern 4 and it’s Gleason 8. Other docs won’t discuss surveillance as an option and if they get RT it’s two years ADT (plus median of 2 years after to recover T)
