I wrote an article re: work/life balance for this months edition of the AUANews. Whatever you have been putting off in your personal/family life, there is no time like the present!
@AmerUrological
Happy to share
@JUrology
has accepted our paper: men with GG2-5 CaP delayed RP up to 1 year after dx was not associated with adverse path, upgrading, or pN+ in the NCDB. May provide some reassurance to men experiencing surgical delay due to

We have an opening in our residency program at
@WayneUrology
. Looking for someone who has completed a general surgery or urology internship at an ACGME accredited program and can start as a PGY2 in July. Please DM me or
@aron_liaw
for more details
Some good news to start the day: I’m happy to announce I am officially returning to
@WayneUrology
and
@karmanoscancer
after finishing at
@FCUroOnc
this summer. Many thanks to my family, friends, and mentors that helped make this possible
I made the mistake of flushing the toilet for my 3.5 year old son. The only way to make the world whole again was for me to immediately pee so he could take what was rightfully his and flush the toilet instead of me as recompense.
Being a parent is wild.
Just finished my 100th ride with
@onepeloton
I’ve lost 30 lbs since July with this magic cardio machine. Big shout out to
@ArcotRohith
for pushing me to get a peloton and
@AllymissLove
for having the dopest rides

As surgeons, we sign on to do good but are vastly underprepared for the emotional devastation that comes with complications and hurting those we want to help. Truly indebted to

Do surgeons have phenotypes? Are some surgeons more consistently aggressive than others? In
@MUSICUrology
, surgeons that tended to use AS for LR prostate cancer also utilized more AS for SRM. H/t to
@mwang50
, Sophie Wittenberg, et al for this effort.
Great job
@AlbertoCastroMD
presenting his work investigating the incidence and timing of adverse events following minimally invasive urologic surgery and the implication on same day discharge.

programs and PDs keep an eye out for this one, applying this cycle!
Privileged to hear
@DrChoueiri
speak on the past, present, and future of adjuvant treatment for RCC
@FCUroOnc
this AM. Very exciting to see what’s in the pipeline and has the potential to be practicing changing precision medicine in the near future.

On Sunday — 2:31:54 in Chicago

Unreal.
I mean, all treatments for prostate cancer have risks, but I don’t think we need to write guidelines specifically on the more brutal ones ;-)

Great article from
@UCSFUrology
on men with GG1 PC which are upgraded to GG2 on AS and continue on AS. Significant portion of men remain free of tx. Low volume GG2 was once a trigger for tx but is now considered appropriate for AS if on the diagnostic or surveillance biopsy
In medicine, there are certain events that are so devastating that we will examine and dissect every part of the system and care episode to figure out how we can try to avoid that event from happening again. Those are called never events.
This should be society’s never event
I feel morally obligated to speak up.

Seeing lots of tweets from folks being very productive at the

Just in time, my last paper published in 2019: which patients with biopsy GS 8 are likely to be downgraded at RP. Collaboration between

with much credit to

Now that every has had a chance to catch their breath a bit, for those that didn’t match and are looking for a research year, consider our position at Wayne state. Opportunity for institutional research and projects with

Published just in time as an appetizer for the
@MUSICUrology
collaborative wide meeting this Friday. H/t
@randy_vincejr
on this great piece demonstrating what we’ve been able to accomplish and how far we’ve moved the needle for AS in Michigan.
Starting next month
@JUrology
will publish the names of peer reviewers and a summary of the peer review with accepted manuscripts. I try my best to follow the golden rule: review onto others as you’d want them to review onto you but this still makes me a little uneasy. Thoughts?
Apparently this is the type of nonsense I can looking forward to with my two boys. Amazing how natural the mischief comes to the 11-month old.

Great news from the ABU today, passed written boards. Congrats to all my newly minted urologist colleagues! Many thanks to all the dedicated faculty that help make
@StateUrology
a great place to train. It takes a village!!!

@AmerUrological
Ready for another day at the
@UroOnc
meeting. That being said, I’m going to have a respectful discussion over coffee with the next person who tells me LN dissection is therapeutic (testis excluded). This event will be live streamed.
Actionable data from
@EinsteinUrology
presented by Curran Uppaluri on natural history, incidence, and timing of recovery of urinary function after acute neurological injury. 💪
Seeing a lot of tweets from PDs and
@SUO_YUO
fellowship programs about meeting up in DC at the
@UroOnc
meeting.
I’m not a PD and do not work at a program with a fellowship, but feel free to DM me if you want to hang. I’m feeling left out.
As I prepare to take oral boards, I’m concerned about this likely scenario:
Examiner: 71 y.o, PSA 8, 3/12 cores GG1 undergoes RP and now has…
I stand up, flip the table, and spew a manic 25 min manifesto about AS
E: Fail, you didn’t answer a single question
Me: worth it
Anyone else really not like when people refer to patients as “customers”? Just rubs me the wrong way, feels like it cheapens the relationship we have built.
Inspiring talk from
@drphil_urology
on a mindful approach to personality and purpose for surgeons at
@FCUroOnc
this am. Great pearls of wisdom for self care. Strongly recommend you check out his podcast: Operating with Zen. Thanks again, Phil!
Hot of the
@urogoldjournal
press, MUSIC experience with confirmatory tests (MRI/genomics/repeat biopsy) vs. no confirmatory test for men on AS. Still a lot to learn regarding effectively/efficiently use of resources for AS.
@MUSICUrology
@arvinkgeorge
Similar adj/salvage RT/ADT for men with GG1 PC undergoing immediate RP or delayed RP after AS. Little/nothing is lost in terms of cancer control for men that choose AS and go on to need treatment

fellow graduation tonight. Lots of people to thank. 2 that aren’t with us anymore are my Dad and Grandfather. Both amazing people that have greatly influenced my life. I honor them by wearing my Dad’s watch (grad gift to him from my Mom) and Grandfather’s cufflinks
Virtual format of
@SUO_YUO
meeting has been pretty successful. Poster sessions and networking/social aspects are diminished but now I can spend my time outside the meeting rubbing elbows with these two shakers and makers. Will a hybrid meeting format develop in the future?
@DGlaucomflecken
Can anyone show me data that prior auths actually reduce unindicated tests/procedures? As far as I can tell, they only appear to be a barrier between the patient and the medically indicated test/procedure.
H/T to
@mwang50
for leading this effort.
@SUO_YUO
and
@UroOnc
keep an eye out for this rising star, he’ll be up for grabs in this year’s fellowship match
We are cool people that do urology. That’s pretty much all you need to know, but if for some reason that wasn’t clear and you have questions, come learn more at our open house.
Best of luck to all the future urologists in the match today, but if today isn’t your day, consider this year long, funded research position with
@WayneUrology
. DM me,
@aron_liaw
, or
@vercnockej
for more info.
I hope the match works out in everyone’s favor, but for those looking to bolster their research experience and reapply to the match next year, we have a paid research opportunity

Cool study but table 3 is pretty concerning. Goals of AS are to preserve QoL and quantity of life. Lots of data showing AS is safe, hard to improve on perfection.
Enza arm: approx 50% fatigue, 30% gynacomastia, 20% ED, 10% decreased libido. That not the QoL I aim for on AS IMHO
Do surgeons have phenotypes? Aggressive vs conservative? Surgeons in
@MUSICUrology
that tended to use AS for LR PC also tended to use AS for small renal masses. Preview below, come discuss more with
