I've just been shown the NHS "Healthy Living with Type 2 diabetes" web page 😬 Highlights include "there is no cure...but there is research looking into this" and (even better/worse) "you need at least 130g of carbohydrate a day". I'm banning it being shared in our practice.
My consulting room carpet has been replaced with something more appropriate for infection control. And just for the record, this is the official reason for my happiness and no other.

Just had a discussion re in what circumstances GP’s would offer a home visit to mobile children with URTI. symptoms. We agreed it was probably in the 1950’s. On horseback. Between a home delivery and a round of golf.
I’m putting it out there: I don’t believe it takes 5 years to get a medical degree. And I do believe that you can learn on the job if you already have a clinical role. Doctors should be honest with themselves.

Pre-diabetic patient. Generally good lifestyle. Not overweight. Pushed harder at lower carb and healthy fats. Result? No longer prediabetic. Slightly raised total cholesterol but significantly improved TC/HDL ratio. Happy patient and doctor 😁

Private GP online consultations appear to consistent of getting paid for having a chat with a patient….before referring them back to their GP to get any actual investigations or treatment. Nice (albeit pointless) work if you can get it. Am I wrong?

Whilst the evidence for Vitamin D specifically in coronavirus may not yet be clear, I think it's sensible to consider supplementation. I've been doing this for years anyway. Every little helps, particularly as people may struggle to get outdoors as much in winter

I’m not militant. I haven’t been on strike in 25 yrs as a doctor, 21 of them as a GP. I will vote for strike action now. Enough’s enough. I won’t peddle the fiction that primary care is doing ok and that a few more targets and a little extra cash is going to solve this.
93% of respondents to our snap poll of GPs told us the ‘rescue package’ proposed for general practice is unacceptable.

Absolutely Neena. I don’t start a consultation until they’ve finished and I pause it if they answer their phone during a consultation. Once I’ve established it’s not an emergency I politely offer them the choice of finishing either their conversation or our consultation.

Home visits should be for end of life care or people who are truly housebound (never leave the house for any reason), with very occasional exceptions. No one expects a dentist or a consultant to visit them at home, so why should a GP? Too precious a resource.

Why is the prevalence of inflammatory arthritis increasing? Whilst I agree that diagnostic criteria, better records and an ageing population may be relevant, the underlying cause is pro-inflammatory factors, the greatest of which is ultra-processed food. No mystery here.

People struggling with their weight deserve compassion. It’s a complex issue, everyone has a different story and pain to deal with. I speak to a lot of patients about this. Need to address underlying issues before weigh-ins, diets and counting calories.

This clearly needs to be settled by a drinking contest. Specialists have to take a drink for every patient that the GPs see and never refer. GP’s have to take a drink for every patient seen by a specialist because GP’s have refused to. It’ll all be over pretty quickly.
Not all medicine is complicated. Some conditions are perfectly appropriate for a pharmacist to advise on and issue meds. Yes, there's a risk associated with this. A good relationship between surgeries and pharmacies is key. It's not perfect. Neither is the world we live in.
I heartily endorse this view Phil. I made the mistake of carrying on with surgery telephone triage and CCG conference calls for over a week before having to admit defeat and allow myself to be sick and give myself time to recover.
If you get symptoms of the virus, & you’re working from home, my advice is stop, even if you feel you can keep working. You need to destress as much as you can, get plenty of rest, stay well hydrated & forget work. Otherwise you risk far worse fatigue in the short & longer term.

With very occasional exceptions a 1* review usually says more about the person rating you than the service they received 🤷♂️ and if any response was appropriate I wouldn’t spend any clinician’s time on it. Our PM once had a review taken down from NHS choices.

Bluntly…it’s total nonsense. Surgeries are open. When I did an audit of walk-in ED patients in Herts most had not even bothered to ring their GP first. I wouldn’t waste time debating it, just correct and re-direct. There are longer phone waits..so some will turn up @ ED 🤷♂️

We have a practice policy that tasks requiring patient contact are not allowed and are turned into an appointment. This means the patient has to wait longer, but we have to accept that we have finite capacity. We must have boundaries and reach the end of a day.

Most patients don’t need A&E. I hope this app doesn’t encourage them to go there just because the queue is short. It’s not like pre-ordering a Starbucks. Consider self care, pharmacy, GP surgery, urgent treatment centre and finally A&E in that order.

Thanks for sharing Nick. I was diagnosed with IBD after a 2 week wait cancer referral. Put on meds. Told by my consultant and IBD nurse that what I ate made no difference. Went low carb to lose some weight…cured my IBD. Off all meds for years.

You may also wish to consider that no heart attacks, strokes or deaths are prevented by controlling mildly raised blood pressure with drugs (140-160/80-100 mm Hg). We'd be better off focussing on prevention and lifestyle measures which have no adverse side effects

Agree. I'm heartily sick of the vitriol I see on twitter. I have no issue with people raising their concerns, and there are clearly issues to iron out and questions re the govt's agenda but it can also be a cover for arrogance, snobbery, insecurity, protectionism and cruelty.

What I’m a little baffled by is the negative response to a pathway from PA to doctor. Are the same people who bash PA’s for not being doctors (duh!) now also objecting to a way to make this possible and overcome their previous concerns?

Fund GP at the level you did 15 yrs ago & the public will get access to an expert generalist who knows them & an NHS that feels safe.
Reward the one sector of the NHS with no IA, no deficit, & biggest productivity with investment, hope, & sustainability.

I didn’t follow their “advice” btw, after d/w patient. Scary to think how many patients and GP’s might just blindly/reluctantly follow this one-size-fits-all approach. Medicine is more than algorithms, pathways & box-ticking.

Indeed. I accidentally put my IBD into remission by going low carb 5 years ago. The start of my journey into learning about inflammation as the common soil for non-communicable disease. When I was first diagnosed my specialist doctor and nurse told me diet made no difference 🙄

You could get one of your team to send them a form to have the test done and cut out the GP middle man/woman...who is otherwise being used for an admin task. Presumably you want to know the result so you can advise accordingly?
