May have just had one of the most soul-destroying shifts in A&E I’ve ever had. How is it ok that people are waiting on a chair for over 2 days for a hospital bed? The moral injury of delivering such poor care is just awful.
If you’re an off duty junior doc who comes across paramedics tending to a trauma patient, here’s a list of 6 things to avoid…
1. Not introducing yourself.
2. Wandering out into the road without any PPE on.
3. Refusing to leave when your help is kindly declined.

There’s nothing wrong with introducing yourself & asking if there’s anything you can do to help. But if your help is declined then respect that & trust that your paramedic colleagues know what they’re doing.

In all seriousness tho, it’s disrespectful to our paramedic colleagues to assume that because we have a medical degree, we know better than them & have a right to tell them what to do when it comes to managing the kind of prehospital stuff they do day in day out….

4. Telling the paramedics how to do a basic A-E assessment.
5. Repeatedly stating ‘yes but I’m a doctor’ when the paramedics tell you they know what they’re doing.
6. Pointing at the female PHEM doc + telling her to go get some oramorph off the ambulance 🫠
Every time someone tells me you can’t give Hartmanns to patients at risk of hyperkalaemia as it can worsen it, I die a little bit more inside.

Hearing of so many excellent doctors not getting an ACCS EM training post this year. It was 2.2:1 competition ratio in 2016 when I applied & now it’s 4.99:1 - not as if the NHS is in need of A&E doctors tho 😵💫
I got my very own playmobil air ambulance today for my 34th birthday to match the RRV my daughter got for her 5th back in Jan 😂
She says I’m too old for it and should give it to her. I strongly disagree.

Being a heavily pregnant CT2 in anaesthetics.
10ml syringe of propofol in my top pocket which was intermittently leaking milky fluid.
Consultant told me at the end of the day that no one had said anything because they all assumed I was just lactating through my top 💀💀💀

I think if you have a pt who is critical (ie cardiac arrest, major trauma) & theres only a couple of people there then there’s nothing wrong with approaching, introducing yourself & offering a hand - holding open an airway, doing some chest compressions, holding a bag of fluids.

As someone who works pre-hospital and in A&E in the West Mids, I don’t know how we’re going to make this work. When we have ambulances queueing up, it’s because we have absolutely no trolleys or spaces to put people. Putting a 45 min time limit on it doesn’t change anything.

Big toe injuries are often managed quite differently to other toe injuries - your big toe is important when it comes to balance and walking whereas your other toes can fall off and it doesn’t make much difference 😅

I struggle to comprehend how someone who is a doctor but, more importantly, a father and a Christian, could ever in a million years contemplate posting something like this - along with most of the stuff you post. Not very salt of the earth.
How am I only just discovering this?!? I feel that as an A&E doctor, but more importantly as a mum, I should have known this. I've definitely given my toddlers a slushie on more than one occasion 😵💫
Did you know that slushies can be dangerous for small children? They contain glycerol- can cause blood sugar drops, loss of consciousness and seizures! Don’t give to under 4’s and don’t let under 10’s have too much (ie no bottomless cups!)
It’s been an absolute privilege to have journeyed through the pandemic as part of the Heartlands intensive care team - what a rollercoaster 9 months it has been!

Fast forward to 2023, I’m an EM reg, just started a job doing PHEM and I now have 2 kids.

I was detained under the MHA due to severe postnatal depression 5 years ago when I was an SHO. Spent 5 months as an inpatient. I’m a senior registrar now and my lived experience of severe mental illness has helped shape me into an even better doctor.

Yep. 100% believe this. I’m an EM ST4 who has never done an open chest drain - I’ve previously not been allowed to do them because “there’s too many patients waiting to be seen, it’s thoracic/resp/gen surg’s job to do the drain”
Male trauma patients are over twice as likely to have TXA administered than female trauma patients - particularly older females, with female patients receiving TXA later on in their journey compared to males. Why?! Unconscious bias?

I remember as a CT1 being made to do my 1st ever RSI whilst 10+ people watched and the consultant ripped me apart and then demanded I write a 1000 word essay on how poor my performance was and how I can do better before then walking out and leaving me close to tears.
Anaesthesia is seen as a welcoming & inclusive specialty. But let’s just talk about an elephant in the room. Bullying. We are not exempt from this.
