Having a little giggle and smirk with your mate on national television during the
#DowningStreetBriefing
when nearly 70,000 people have died and my NHS colleagues are working themselves to the bone supporting critically unwell Covid-19 patients. Absolutely disgraceful. Dire.
Did I just hear that correctly?
When the Critical Care unit I work in was filling up with Covid-19 patients in March 2020, Carrie Symonds interrupted the governments pandemic response by demanding that the press office focus on addressing a story about her dog?
#dominicummings
On nursing strike action...
Seen a few tweets from other healthcare professionals today saying 'we will step in to cover our nursing colleagues' - however well intended, the narrative that anyone can do nursing, is exactly why pay has been so poor for so long.
Any answers on
@BBCRadio4
just now...
'Nurses should learn on the job...'
'Nurses don't need degrees...'
'Nurses get £600 a week but I only get £150 state pension'
'We need more cottage hospitals with lower-skilled nurses...'
The callers on this programme are completely deluded.
Here is my attempt at an entry-level blood gas analysis tweetorial for those new to critical care...
Here's our typical normal values...
pH 7.35-7.45
PaCO2 4.5-6kPa
PaO2 10-13kPa
HC03 22-26mmol/L
Base Excess -2.5 to +2.5 mmol/L
1. What's happening here?
Dear Panic Buyers
I'm on my way home from a 12hr nightshift in the critical care unit. Stop worrying - there's plenty of food.

I'm a Registered Nurse. Can we please stop perpetuating this hero bollocks. Let's not frame any pay rise as a 'reward' for the pandemic response - it's about simply paying people enough money to save for a mortgage, have a holiday, to put petrol in the car, to buy a travel pass.
My 12 year-old daughter had a first attempt at painting with the brushes and palette that belonged to her great grandfather. She thinks it's 'rubbish' but I beg to differ.

@Otto_English
I'm working another twelve hour night shift in our busy ICU. I'm sat on break viewing this video - a few feet away through a set of double doors are several critically unwell Covid-19 patients. These jokers have absolutely no idea. None of my colleagues find this funny.
Nurses don't want a 'one-off' payment.
Nursing professionals want a sustained increase in pay, that aligns with inflation - pay that reflects their important skill set and societal worth.
I'm a Registered Nurse. I use public transport frequently. The striking rail staff have my complete support. All workers private and public must stand together in the struggle for improved pay, terms and conditions.
#RailStrikes
Let it be said that
@patcullen9
has done a tremendous job in galvanising RCN members and following through with industrial action. Today's recommendation from
@theRCN
to vote in favour of accepting a 5% pay rise is in complete contrast to recent RCN pay rhetoric. What happened?

Critical Care is a massive team
I'm relatively new to nursing. Genuine question... at what point in RN history did RN's adopt the position that they have to accept Nurse In Charge leadership duties at Band 5 for no extra pay? How has a situation evolved where this responsibility is not monetarily rewarded?
Paused yesterday for a moment & observed some of my colleagues quietly donning their PPE prior to entering the Covid-19 area of our ICU. No hesitation. No complaining. These people have been doing this continuously on most shifts for over 18 months now. Let us reflect on that.
Ironic to see the clamber today from government & chief nursing officers regarding strike derogation and the need for safe staffing in specific areas - on most days, in many clinical areas, unsafe levels of staffing & the resultant suboptimal care are tolerated as the norm.
Nurse education UK...
3 yrs of study
£27k+ student debt
2300hrs of clinical placement
And still, upon starting in post, you cannot flush a cannula or give a single tablet without additional local competency training.
Are there any other countries that operate in this way?
Student nurses...
There will be times when everything seems impossible, when assignments stack up, a dissertation looms over you, when placements are exhausting, when you struggle financially, when your social life vanishes. Don't give up. Talk to someone. You can do this.
When you're a Band 5 nurse in the early years of your career there's a degree of anxiety when you see people around you progressing to advanced practice, clinical academic routes, leadership posts etc. Remember though - your role as a nurse is just as important & you are amazing.
@campbellclaret
As someone who worked in the ICU all through the peaks of the pandemic I have found this entirely outrageous saga morally injurious. No 10 repeatedly flouted rules whilst my NHS colleagues were overwhelmed with critically unwell patients. This I can never forget or forgive.
One of the hardest aspects of being a Registered Nurse is seeing patients who are seriously unwell because of neglect, loneliness, a punitive benefit system and inadequate social care provision.
Frightening how many tweets I see from student nurses who are being used as pseudo-healthcare assistants whilst on placement. Even more frightening is that some senior nurses seem to think that 'mucking-in' by washing dishes & emptying bins is appropriate use of placement time 🙄
@LissaKEvans
Why is nobody challenging his behaviour? Why haven't the hospital leadership insisted that he follow their policy? This has really wound me up.
Thoughts. Would you expect student nurses to know, understand and be able to use these terms in their second year of study?

....this is all it takes to make a student nurse feel a million times better. Anyone can do this.
All of the student nurses I have met recently have been really clear about where they don't want to work once qualified. There appears to be a shared intelligence network influencing employment choices driven by accounts of placement experiences.
Occasionally see student nurse posts on twitter saying how miserable placement has been ☹️
Well today I met a student who was absolutely beaming because our consultant repeatedly used her name, invited her to get involved with lots of tasks & bigged-up a career in nursing 🤩
What does the future hold for graduate-entry nursing course numbers?
Why start at Band 5 as a Staff Nurse after a two-year graduate-entry nursing course when for the same study commitment and loan debt you can go straight in at Band 7 as a Physician Associate.
Someone just got on the bus and started putting these anti-vax, covid-denial leaflets on the empty seats. So before getting off I picked them all up and deposited them straight in the bin. The chap was not very happy.
Why does some NHS mandatory training insist on testing recall of information that makes little to no difference in how you might respond in the clinical context?
For example testing your ability to recall section numbers of a parliamentary act? What does testing this add?
Johnson today on his trip to a hospital. Wandering about with no mask. Is he exempt? Why aren't the hospital leadership challenging him about it? Incredibly bad optics... Personally I find it galling.

Until there is change, school leaders need to collectively refuse to engage with Ofsted.
Don't answer emails.
Don't answer calls.
Don't meet them.
Don't furnish them with data.
Don't engage them at all.
Highly unlikely though in a system predicated on fear & ruining careers.

As a Registered Nurse I feel truly ashamed that this happened - awful & unnecessary. I cannot understand why some people treat colleagues like this? Insecurity? An attempt at exerting control? This 'shouty' nurse needs an NMC referral and sacking.
The NMC rhetoric :
'The intention is for Nursing Associates to support, not substitute... '
The reality:
31 full-time Band 5 Registered Nurse posts replaced with Band 4 Nursing Associate posts at Surrey and Sussex NHS Trust.
One patient extubated
One patient stepped-down from critical care.
A much-needed psychological boost for the team. A small victory in this fight.

I'm a critical care nurse. Until 2016 I used to be a secondary Head of Year. If a child has a parent or sibling with confirmed Covid-19 and they live in the same household then it is, in my view, completely negligent & grossly irresponsible to allow them into school.

I'm an ICU nurse here in the UK. All sensible mitigations have been abandoned. We're instead sold a mistaken narrative of 'freedom' and 'personal responsibility' along with rose-tinted 'British spirit'. The situation is a disaster.
What student nurses need on placement...
1. A mentor who explicitly teaches
2. Encouragement and praise
3. Weekly goals aligned with competencies
4. To leave each day and have learned something new
5. Time to repeat and consolidate skills
6. Door codes!
Any more folks?
Just listened to Mishal Husain's interview with Tory immigration minister Michael Tomlinson on

I will not be accepting this offer. We should continue with thr industrial action and coordinate a response to any settlement with Junior Doctors.
I'm a critical care nurse. If I'd hosted a booze-up with work colleagues during the lockdown I'd have faced a disciplinary & sanction from my professional regulator - I would also immediately resign as my actions would bemorally indefensible.

Can I ask. What is it that is preventing you from having the vaccine? If you became critically unwell with Covid-19 and required intensive care in order to save your life, would you decline the science behind that too?
What I've learned...
When someone says 'you won't need all that kit for your trip to CT scan, your patient is fine on 2L'
Ignore them, listen to your inner voice, and take all the kit.
Why...
1. Why do nurses write 'evaluations' for patients? I sometimes read them from pts admitted to ICU - often simply a list of tasks done according to protocols requiring no independent decision making - may as well just write - 'I followed all the pre-printed care plans'...
Tomorrow is my first day as a Registered Nurse... 🙂😮😬
I have the 'night before starting a new placement feeling' - except this time it's the real deal. Met some of the team on Friday and I was made to feel very welcome.
@TomHodden
Can't stand this 'poor veterans' whataboutery. I served in HM Armed Forces. There is ample access to support, resettlement allowance, education. I don't buy the argument that ex-service personnel are more worthy/deserving than those genuinely fleeing poverty, war, persecution.
A nursing post was shared on here earlier where the workload was explicitly described as 'overwhelming' therefore 'resilience' was an essential applicant trait.
Incredible that this institutional violence toward nursing staff is so normalised that it features in the job spec.
@bbclaurak
I'm a Registered Nurse in critical care. I can say with total confidence that the departure of Gina Colandangelo will make zero difference to my role.
Student nurse, it's tough.
But you must.
Get the hours in, sit on the bin.
Handover, no seats left.
Assignment due, social life bereft
Help with a wash, struggling for dosh, it's a test.
Student nurse, you'll get through.
Upload. Turnitin.
Don't fret.
Your best yet.
The Year 11 prom was cancelled but my amazing former colleagues took it to the doorstep today visiting students and their families to say goodbye. Absolutely brilliant.
@HillsToClimb
My Poole pottery set is at least 50 years old and still in daily use. I picked up an entire service for £20 years ago at an antiques shop in Stamford.
Well... 18 months ago I left my job as an experienced teacher & Head of Year. I started a graduate-entry programme in Adult Nursing. In 6 months time I will be a registered nurse & I have loved every minute of it so far. If you are considering a career in nursing then go for it!
Idea: a small conference about nursing called 'Nursing: New Voices' - whereby all the speakers are nurses who have not yet had a platform or status that allows them to engage with a wider audience? No nursing grandees or established speakers. Who fancies helping me organise it?
Can you do an ECG?
Yes sure.
Can I actually get the NHS printer to print something without having to apply to the Open University for a degree in Computer Science?
Er no.
@naomicfisher
@RaeSnape
These policies are adversarial and grossly punitive - can teachers not be trusted to apply their professional judgement in dealing with these minor issues? No pen? Give the child a pen, move on, get on with the learning and foster a good relationship - don't criminalise them.
Drug calculation thread for students...
1. I worked with a student recently and we had an interesting drug calculation that I thought I'd share. Our patient is prescribed 360mg of Voriconazole IV...
Funny that...
Johnson, Prime Minister, fully vaccinated has close contact: Staying at home, unavailable for a week.
Me, Intensive Care Nurse, fully vaccinated, has close contact: Do seven days of lateral flow tests and carry on going into work and performing your duties.
@noodles_nood
Extracts from 'Life, Death and Biscuits' by Anthea Allen - remind me never to buy this book. The snarky comments about medical colleagues are abysmal and are not representative of my nursing colleagues. Attitudes like this don't belong in a modern professional workforce.
Nurse education needs radical change. We have a preregistration education system where 'hours served' & unmoderated subjective judgements against fluffy generic statements are prioritised over objective measures of knowledge, understanding & clinical competency. Change needed.
@jaydbint
I find it hard to believe that any nurse would do this - or do I? I would formally escalate this to a senior nursing lead so that this nurse can access some additional training around the correct use of the Datix system and some help with interpersonal skills development.
11. I hope this entry-level tweetorial has been useful. I've probably made an error so do let me know if you spot anything dodgy. Remember - blood gas analysis is just one tool in the holistic assessment of the patient 👍🏼
Task-led nursing care?
1. There was discussion on here recently about 'task-driven' nursing. I've been reflecting on this. As a NQN I was very task-oriented, reliant on set timings for tasks, doing & seeing things in isolation. This kept the patient safe but was it optimal care?
@thacker_paras
'Staff are encouraged to report colleagues...'
Note to self - never ever get a job in a workplace like this. Nursing staff who put up these nasty little laminated notices need to really think about the culture they're creating.
@emergprehosp
Why can't shifts 'in charge' be paid at Band 6 rate? If you're performing the role then you should be paid accordingly? When did accepting additional responsibility with no additional remuneration become normalised?
@Bethan_Southern
@DurhamCouncil
This advert is terrible. Effectively advertising for someone to bully vulnerable children with complex emotional, social and behavioural needs. On what evidence are you basing this nightclub bouncer style approach?
I was on the picket line. A gentleman appeared and gave me a big bag of chocolate biscuits. He explained that he had recently been diagnosed with a life limiting condition - he wanted to pass on his thanks to all staff at Nottingham City hospital who had been supporting him.
Today I worked with a newly-qualified RN who had been redeployed to help in critical care. The last time we met was approx 10 yrs ago when he was a 13 year-old pupil in my class! Both of us spent the day together working as a team caring for a critically ill patient. Fantastic!
I've seen several tweets from senior NHS bods & ministers who attended this. Individuals & organisations have asked if there was any nursing representation - I've not seen any response yet. The ongoing silence speaks volumes about how the contribution of nursing staff is valued.
Today I hosted the NHS Recovery Forum in Downing Street with clinical leaders, health experts and ministers.
We are determined to ease pressure on the NHS, ensure better care for patients and deliver our promise to cut waiting lists ⤵️

No - just skilled people who've studied hard and committed to a public sector career wanting a wage that reflects their societal worth. Can't stand this constant 'ministering angel' narrative that negates adequate pay.
If you are a student nurse with nagging doubts about your chosen career then please persist. Get that first post, absorb all the new learning - so many brilliant and varied opportunities for you - skills, further education, adv practice, leadership etc. Nothing more satisfying.

Very interesting study: Nurse 'worry' & 'pattern recognition' has a higher predictive accuracy of acute inpatient deterioration than most published EWS scoring systems. Accuracy rates were significantly higher for experienced nurses.
@iDrSunny
As a Registered Nurse I find comments like this embarrassing. I don't care how long you have been a Registered Nurse or how many degrees you have - there is no parity with a doctor that has devoted many years to studying medicine.
Started my eight-month rotation in critical care at the Queens Medical Centre today. Fabulous team, comprehensive induction session, a great welcome and I'm really looking forward to getting involved, developing new knowledge and skills. Being a Registered Nurse is a great job.
