My BSHT summer studentship with @LabMutch @gael_beverley @nicola_curry was an absolutely amazing experience, and the opportunity to present @BritSocHaemThro #BSHT22 this week was awesome! I can't thank the BSHT enough for what has been the highlight of my academic career so far!

I had a truly horrific first year at medical school – many untweetable things happened to me – and so I am *extremely* relieved to have come below average in my cohort and 'only' passed my exams this year.

Not the medical school threatening me with the GMC because I asked them why we didn't have a timetable, and apparently this contravenes domain 3 of achieving good medical practice ??? 👀

@DrCanuckMD Before immediately dismissing the student, I'd be interested to know *why* they thought intubating this patient was a bad idea. If they mentioned something specific and reasonable, I'd be grateful... said student might have noticed something others didn't.

Not all of Oxford is posh. When you say Oxford is posh, you reinforce the stereotype that you have to be posh to go to Oxford, and fewer not-so-posh state schoolers apply. Yours, someone who didn't want to apply to Oxford because of people like you telling me I wouldn't belong.

Anyone who suggests single rooms only everywhere has never: cohorted a bay of confused patients themselves, seen the camaraderie of oncology (/other) patients, or seen how much harder ITU breaks are with (non-attached) side rooms... I have strong opinions on this one tbh

Yet another decision from our Lords and Masters that goes against the evidence! Yes, single rooms reduce infections and might be more flexible. BUT they create work for staff and many older pts actually feel less safe and more lonely. More expensive!

Peak NHS "Were you aware of your appointment on 6th July?" "No? Usually I get text reminders when things are booked as I am not always at Y address" *sighs in student* "Oh our text service isn't working" And nobody thought this might be a cheaper fix than many missed appts...

@VirtueOfNothing This is utterly ridiculous. I often use that exact piece of road as a driver... 28mph is not an 'unsafe' speed for that road, even during peak times... A 12 year old should reasonably be expected to not run out into the road... Absolutely nonsensical judgement.

@timricketts_ Yep 100% agree. I find it wild that it is entirely possible for the bank band 3 HCA to earn more than the ED SpR in charge OOH... now the HCA's pay might be reasonable, but the doctors certainly isn't... I'd earn more doing full time bank as a HCA than I would as an NHS FY1.

Medtwitter -- can we talk about the reality of being stuck in a specific city for 5+ years for medical school, with almost 0 chance of transfer to anywhere else, no matter the circumstances behind it... The choice is stay, or leave medicine entirely, and that hurts.

THIS. Length of stay means that every COVID ICU patient effectively takes up a whole cardiac ICU worth of resources (capacity around 10-20) over their stay + cancels a lot of elective cardiac surgery

48553 cases today. 100 more patients in ICU with covid19 than 5days ago. No one talking about covid length of stay so I will. Median ICU stay:- Heart/major cancer surgery 1-2d Covid (not ventilated) 5d Covid (ventilated) 20d Covid (kidney support) 32d Do the maths

Was sent a certain thread by colleagues. The task-and-finsh group meeting I attended *at age 16 in September 2018* as a voluntary patient representative with a complex medical history and many interactions with the NHS. My role was to give input in this capacity.

@fuzzymittens I really don't want it to be, but isn't this a bit of a 🚩for someone who drives an Audi being in your house without you knowing, and accidentally [or deliberately...] leaving their keys? Many people have 2+ keys...

These are all medicines required to keep ONE covid patient safe for ONE day one critical care…. Or just ONE vaccine? #getthejab

I cannot more passionately disagree with this. Healthcare is about teamwork. Medical students who think they can just "be nurses" for a day... no. Just no. Calling patient care menial... not a good take.

Day 4: finally ✅️ paddleboarding Didn't fall off once... why? Not because I'm good, but because jellyfish are an *excellent* incentive to NOT fall off

Spidey sense at ice skating tonight. Open phone after 75 mins on the ice + parents both have COVID... Rapid instructions on how to use the pulse ox... 💔 they're okay atm but still so so scary Feel extremely guilty for going on holiday now Starting to think this week is cursed

@BeckyPlatt3 One of my fave supervisors often signs things off with BW! I take it as a sign of a bit of a relaxation of formalities which isn't a bad thing!

@rchrdsn_ Without explicit permission I don't think this is acceptable? Especially if they don't know if sensitive information might be mentioned or not as this is clinical too...

@EMBoardBombs Plasmalyte for the win Abnormal saline is outdated for almost everything ... Plasmalyte makes things better faster but most of the studies are too small to prove benefit in terms of mortality etc

@palmd Controversial, but perhaps they are the ones that *should* be prescribing it. Yes, it is a nasty drug for the average patient, but it does have a place for refractory pain that *might* respond to it + many patients do individually find it helpful

@RobJimFleming @DrAdamLawton @Dr_DeanS Absolutely this. I couldn't disagree more with notion that everyone knows what they're getting into -- really... and especially after a pandemic and how *much* worse things have got, they should be able to leave and not feel guilty

@DrANChekroud Some departments are utterly, utterly broken now... not the fault of the individuals working there, but a combination of the sheer pressure the system faces and a continued [poor] strategic decision at a national level to centre risk in ED waiting rooms and prehospitally...

It is concerning to me just how many doctors and other clinicians are willing to actively bully me on here and actively attempt to harm my reputation, as well as undermine a *patient's* contribution to an important piece of work. This *is* bullying, and needs to be called out.
