Lovely to see the ‘junior doctor’ being scapegoated for Kerry CAMHS scandal on RTE news tonight.This was a failure of the HSE - no supervision, no help, noone to discuss cases with, no permanent consultant yet it’s the junior doctor at fault! Anyone fired yet? Anyone resign?

Had a long tough clinic today. Broke some bad news to a few patients. Most striking thing however was how many older patients just wanted to chat and talk. All been cocooning for many months, missing family, grandchildren and social contact - today was a day out for them. 1/4

This lovely Curlew turned up in a suburban garden In Blackrock yesterday. No injuries but rather emaciated and underweight. I managed to gently corner it and pick it up. Taken in for assessment/rehab by @WildlifeKildare - they are a fab resource well worthy of our support!

They live in constant fear of COVID. Two patients had their partners diagnosed with terminal cancer and now face even further isolation and loneliness. This pandemic has so many unrecorded and under recognised consequences that affect our parent and grandparents. 2/4

They are really struggling to thrive mentally and physically - another patient with MS has seen her mobility deteriorating without her physiotherapy. It was a tough clinic for me but this pandemic is so much tougher for so many others. The sooner we reduce the infection rate 3/4

@Caolan1983 @HSE_HR I suggest give them 24 hours to pay and if they don’t reimburse you - you go off call until paid. Cc the Head of HR, the CEO the Medical Director and your Consultant. Also contact the IMO and ask for assistance. There are others in same boat!

@CoachCaoimhe I reffed a J3 match once when a player got a head knock and was clearly unsteady. His team physio said he was fine to play on and tried to argue with me until I pointed out I was a doctor with a level 3 sports injury qualification as well as being the ref! He went off!!

Not a campground! The vegetation is protected legally. An occasional camper is fine not these numbers!

@DGlaucomflecken Had a patient die in hospital after 30 days of difficult palliative care for metastatic prostate cancer. Insurance form filled out fully indicating patient died! Company wanted outlier report! Report done and forwarded. It read: He was very sick and he died! Claim paid!

@IrishRainforest Large herds of goats are a huge problem in Africa. They eat all vegetation to the roots, strip bark killing trees and leading to desertification. Every time I hear a charity pushing goats for Africa I wince!

@AshleyGWinter Commonest cause of testicular pain I see,a major cause of sensory urgency! I have 2 pelvic floor physios I work with - I keep them busy! Seeing loads of it in the IT industry - those guys sit at there desks all day with poor ergonomics. Back, testicular pain, urge, frequency!

@jim_demps Take the rose-tinted specs off Jim. It’s standard clear out but Jack’s leg is planted in the 5G and can’t slide like grass. I also think his wrap missed and pitched him forward and makes it worse. Hugely unfortunate but not deliberate. It’s nowhere near a Croc roll either.

@philipnolan1 As a urologist I wish all toilets were unisex. I see far too many ladies hopping from foot to foot in a big queue. At rugby international in Rome all cubicles in the mens were occupied by women! Overactive bladder is a huge problem for many men and women. Any port in a storm!

Very aromatic nose. Floral rose petals. Crunchy red fruits raspberries and loganberries, some Christmas spice. Very appealing. Ripe red berries, spicy, fresh acidity and ripe well intergrated tannins. Crunchy and juicy. Nice hit of minerality as well. Delicious @64_Wine

@OGdukeneurosurg These are small kidneys for APCKD and why remove the bladder and ureters. It’s common to remove kidneys before transplant but not the bladder. There’s something else going on here! And you don’t transplant someone in the presence of an active malignancy!

@NiamhLQB Well said Niamh - I can’t imagine there is a GP, NCHD, front line medic, nurse or paramedic that’s not feeling the strain of a tumultuous year - and still we’re staring down the barrel of a gun again. Burn out is a real risk for all of us ...and that will impact patient care.

@AshleyGWinter Happens in men as well! Pelvic floor problems are often diagnosed as Chronic prostatitis or epididymitis. Testicular pain is epidemic in IT workers due to poor posture and back and pelvic floor issues. Also leads to sensory urge, frequency and urethral irritation!

Got dinner @woodruffdublin again tonight and got these wines to accompany the meal. Super fresh crunchy Pinot & Luce from Domaine de Montcy. The latter a new grape for me 100% Romorantin complex nose, herbal, hint of honey & apples,fresh acidity rounded palate delicious!

@cihalpin Surely a simple central registry of NCHD details wound obviate the need for all this! An IT student could build that database in an afternoon! If the passport office can manage it remotely why doesn’t the HSE sort it out! It would save millions in duplication!

Went to @VolpeNeraDublin this evening! Fantastic meal as expected but the wines by the glass was next level! Really brilliant! Star for me was Domaine Baumard Quarts de Chaumes 2008. Not often the dessert wine wins out for me! Riffault Sancerre, Domaine L’Ecu Syrah Huet Demi-sec

@theHALLofEINAR @MarkAvery As a urologist I have seen and smelt all those things on numerous occasions and Hawthorn does not smell anything like those - it’s actually a nice floral smell! Who come up with this? Have they ever smelt gangrene I’d bacterial vaginosis? I’ll excuse the bad breath bit!

@McGlackenByrne @drmarkmurphy Time for NCHDs to with draw cooperation and work to rule. No more gap filling. If you’re not meant to be on call- you don’t do call. It’s not your responsibility to fill gaps in rosters and rotas. It’s HRs job to find doctors to fill the gap.

@AlltheInternet @NewstalkFM @ShoIreland This is nonsense. Organs only go overseas if they cannot be utilised here. We receive organs from other states for transplantation in Ireland. Likewise the vast majority of traffic deaths are not suitable for organ donation. Do a little basic research before scaremongering!

Was sitting in the Aviva watching Ire vs Ita last night Half way through 1st half I heard a Peregrine Falcon calling over the crowd noise. It was persistent & I half thought I was mad but then 2 birds flew out and over the Havelock Sq end! Amazing!!! @EricTheBirdman @SoundsIrish

@laoiseman @CrickeyYard @SimonHarrisTD Not if it means abandoning continuity of care to my pre -existing patients who I have looked after for 19+ years many of whom need ongoing care for prostate and bladder cancer. The Medical Council would think dimly of that. There is no public pathway other than A/E for them.

Deep purple-red no hint of ageing. Remarkable colour for 2002. Nose somewhat restrained, red & black fruits,cherries, logan berries, some herbaceous notes, a little garrigue. Palate fresh and juicy, lovely balance, raspberries, cherries, loganberries, tannins fine and restrained.

Another super wine this evening Nascor - a play on Cornas! 100% Syrah from Matthieu Barret. A natural wine. Red and black fruits, ripe tannins with some rusticity. Very enjoyable with a nice rib-eye this evening. @GreenManWines1 @Laurakp1974

Another super wine from @lseabrawine ! Kept for 3y under a layer of flor! Fino like nutty nose but very complex ripe and rich palate with long finish. Floral and white fruits, ending with some nuttiness! A unique wine and absolutely superb! Delicious! 👏👏 @64_Wine @Blinderswine
