If you are a knee ligament surgeon, repair every single one! We grossly underestimate their biomechanical value to knee stability in every plane! Fellow and I will have done 80 primary acl at the end this month; he has yet to see me do a meniscectomy! Please save them!

30 plus years as patellar tendon acl surgeon, 2 metal screws. Most predictable outcome I can promise athletes and parents. Have never seen osteolysis. Easy revision, why change😳

Most importantly on every ACL you do, you must have the equipment/instruments to repair that root tear. If it is primary ACL you get one chance to make it perfect! If that ACL recon fails, knee is very rarely ever back to “normal”. You must get everything perfect the first time!

100% correct, I teach our residents and fellows that you should plan the “revision acl” at the primary! Ie don’t use some new fancy fixation device that you cannot revise simply! I will die on that quote as well! Many of these devices should have never made it to the market!

When Randall Cobb tore his meniscus in 2008 while playing for @UKFootball , he had a big decision to make: remove his meniscus through a standard surgery or undergo a complex repair. He chose the latter. Fourteen years later, he's still in the NFL.

Equal amount of time and effort should be spent on eccentric hamstring work and hip ER/abductors. Everyone focuses on what we can see. Need to focus on what we routinely don't look at!

Best thing about @proatc when him and I worked closely everyday at Pitt sports med, very simple, people skills, which is very difficult in our industry, and never ever comprise care for the athlete for anyone! Easy to say hard to do! I knew in 1992 he was special!

Reverse parrot beak tear into the root is the most common tear you will see as an acute acl surgeon, next is Ramp lesion, fix them all! You under appreciate how they help your acl reconstruction! Fix everyone!

Meniscus pearl: if you see a small radial tear in the body of the lateral meniscus after an ACL injury, be wary of a significant LMORT that will require a repair. They often occur concurrently. Examples below:

What is hard for people in our industry is that the people skills to lots of different people, athletes, agents, coaches, parents, requires a complex skill set! Once you get through that landline, always just do the right thing for the athlete’ let the chips fall after that!

I 100% agree with @MDMillerMDUVA . Hamstring contraction helps the acl deficient knee, why take it. Early evidence starting to come out to use quad tendon if truly physes open. @ACLrecoveryCLUB @ACLSupport @ACLimpact

100 percent correct! All about anatomical closure of the tendon and peri tenon , careful bone grafting the patella, incision is medial border of tendon , and must get normal PF passive motion by week 4. Simple! Never a problem in last 30 plus years!

Roughly 1 out of 10 people end up with kneeling pain 2 years after ACL-PTG surgery. Wish we’d stop using this as an excuse to use other graft choices. My patient’s don’t get AKP and the subsequent kneeling pain is relatively rare.
