The NHS has gone from ‘junior doctors must rotate regularly in order to gain breadth of experience in their training’ through to ‘there’s no point investing in our junior doctor staff, they’ll be gone in a few months anyway’ & people really cannot see the problem 🤦🏼‍♀️

@juliaisobela Arrived midway through a laparoscopic surgery (thanks, timetable). Allowed in but not acknowledged by surgeons (fine). Decided to try and teach something to the Y1 student nurse who was also shadowing; confidently pointed out the uterus and Fallopian tubes. The patient was male.

5. If your pay has suffered due to inflation, you absolutely deserve pay restoration too! (6. But doctors’ & other healthcare workers’ pay has suffered the most) 7. The govt can end the strikes any time they want to. The fact that they haven’t, suggests they don’t want to.

8. Our ‘gold plated’ pensions were transmuted to bronze (read: reduced massively) back in 2015, & most of us will never see their full value (9. The Govt pilfered £4bn out of the NHS pension scheme this year) 10. Yes, we really do start on £15.50/h. Read the darn payslips.

The thing I hated about medicine & surgery was working with consultants who were stubbornly unfamiliar with the tasks on a modern ward. Giving unworkable or downright unsafe plans because they didn’t know/had no interest in finding out what’s actually feasible in the modern NHS.

I have just drunk two Sports Direct mugfuls of tea. I weigh 73kg. Given that one mug is 568 mL (aka one pint), and tea is roughly 1.1g/mL, I have drunk 1.25kg of tea. I am currently 1.7% tea by weight 😂 #Brit 🇬🇧

@DrTADM I do recall on one occasion seeing someone in ED who I’d seen multiple times in GP (F2F and telephone), who declared he hadn’t been able to get a GP appointment in months hence coming to ED. His face when I told him I remembered him from the multiple consultations we’d had 😂

Tonight, if I’m lucky, there will be two senior house officers (doctors with between 2 and 4 years’ experience) covering medicine, due to a combination of rota gaps (not enough doctors to fill the slots) and illness. Minimum staffing is five. (We are also short on registrars).

Thanks to everyone who has sent me this. Patient discharge is a clinical procedure. There is risk if its not done well/completely. The focus on "flow" means its simply seen as part of a logistics chain now. Turning into a competition I really dont know where to start with. 😬🚩

One hell of a week! Outcome 1 at ARCP, then been placed on the Emergency Care pillar of practice with SJA (meaning I can get back into clinical practice again), then finding out I passed MRCEM Primary & today I finally bought my new car. Definitely time for celebratory bubbles!

So NHS HEE wants me to have instructor status - or at least to have been recommended - in ALS, to apply for ACCS core training. When I did my ALS course I was told they don’t recommend med students or foundation trainees for instructor as too junior. 🤷‍♀️🤷‍♀️🤷‍♀️🤷‍♀️

Tried accessing community mental health support a while ago. When I got to the top of the list, I was told I had to commit to a tel appt slot at exact same time & day every week, between 8am-3pm Mon - Fri. No exceptions, no skipping weeks whatsoever, no OOH slots. (1/?)

@pepemac27 I had this on weekend Acute medicine on call, and they refused to do it. Also carrying crash bleep, no jokes, got told “not my problem love, come back Monday. If you really need to, you can just break the glass panels”. One of the few times I’ve actually bothered Datixing.

@drphiliplee1 Got Datixed because I didn’t answer a bleep that doesn’t exist. When I explained that the number they bleeped doesn’t exist, and that the correct bleep number was in fact on the wall next to the computer, they said it wasn’t official because it wasn’t laminated.
