The extraordinary burden of culture-negative #sepsis on full display in a recent article in CCM. Tetsu et al show that across 147,061 community-acquired #sepsis patients, only 14% had positive blood cultures, and another 14% had positive cultures from another site.

Incredibly sad news. We lost a #PedsICU giant yesterday. Hector Wong, MD passed away unexpectedly. He was division head at Cincinnati for over 20 years before stepping up to interim chair. He leaves a massive legacy of sepsis research and trainees spread around the world.

Wow, popping open a LAMP assay for manual lateral flow will absolutely paste the entire lab with amplicon. Get ready for TONS of false positives; just pointing to a "pre-test" and "post-test" area won't be enough. Great work trying to get testing out to the masses, but yikes.

Explaining severe #COVID19 as a 'cytokine storm' or 'genomic storm' is great as a public explainer but also shows how #sepsis still doesn't have an easy explanation. Better definitions --> better understanding --> better funding --> more progress!

Until then, the authors are absolutely right - some sepsis has no SIRS, lots of SIRS isn't sepsis, and treating clinicians should be empowered to do what's right for patients!
