The very rare POEMS syndrome (Polyneuropathy, Organomegaly, Endocrinopathy, Monoclonal protein, Skin changes) can be associated w/ severe pulmonary hypertension - here in female pt. presenting w/ massive edema & pericardial effusion (thx

ESC 2019 Guidelines on Chronic Coronary Syndromes just released (). Finally, Class I indication for coronary CTA in suspected CAD! Text says wisely: use testing according to pre test likelihood, patient characteristics and expertise.

Sometimes it is just nice to see something nice to look at .... cardiac CT always a good candidate.

In myocardial bridging of the LAD, the vessel can have an extremely close anatomic relationship to the right ventricle - may seem to run through RV lumen. (some say "never stent a bridge, may perforate into RV"- I´ve seen it once).

We call it the "C" view in cardiac CT... nicely shows the entire RCA which often runs pretty much in one single plane all the way up to the take-off of its distal branches. Nothing spectacular here, just looks nice.

Review on leaflet thrombosis following TAVI/TAVR in EHJ:
Lack of normal sinuses "flush" after division into two spaces by TAVR may contribute.
Hypoattenuated leaflet thickening (HALT) not necessarily hypoattenuation affecting motion (HAM).

Spontaneous applause as the first result slide of DAPA-HF is shown in Hotline I at ESC Congress 2019 in Paris by John McMurray, signaling, as later suggested by discussant Marco Metra, the start of “a new era in heart failure treatment“.

New ESC Chronic Coronary Syndrome Guidelines just out. For LM stenoses w/ low Syntax Score, both surgery and PCI - as long as full revasc is possible - have Class IA recommendation. Image-guided PCI has Class IA for LM / bifu / long lesions

Atrial Septal Aneurysm (ASA) in cardac CT. Usually defined as excursion > 10 mm (sometimes > 15 mm) to either side. Prevalence in the population likely around 2% (higher in populations with stroke of possible embolic origin).

For us cardiologists always good to see what a heart really looks like. Just came across this video of a heart with 1-day old anterior STEMI showing "inflammation, edema, akinesis, myocardial hemorrhage" (thank you
@BobbyYanagawa
). I rotated it by 180°.

While not an unrestricted supporter of using coronary CTA in patients with coronary bypass grafts, I'll immediately admit that this one looks very nice.
(IMHO native arteries in post CABG patients often too severely calcified for reliable evaluation)

"Less is more"... with higher resolution, impressively little partial volume effects ("blooming") of calcification in coronary CTA on Photon Counting CT reconstructed @ 0.2 mm resolution. Less partial volume effects should allow more accurate assessment of lumen.
