Anatomy Of The Lungs๐
๐Lungs
โขRight lung = 3 lobes
โขLeft lung = 2 lobes
๐นPleura
โขVisceral โ membrane that covers the lungs
โขParietal โ lines the inner wall of the pleural cavity

๐บCuff Pressure
โขMost endotracheal tubes have high-volume, low- pressure cuffs
โขCuff pressures should be kept between 20 to 25mm Hg or 25 to 35 cm H2O to maintain tracheal blood flow
โขMinimum leak technique is often used to determine appropriate cuff inflation

๐Quick information
Morphine and pulmonary edema
morphine reduces preload
Allows more blood flow, lowering pressures -> Allows the heart to work more effectively -> less fluid gets stuck in the pulmonary structures.
Morphine doesn't relieve the edema, but can help manage it.

*Acute Respiratory Distress Syndrome(ARDS)*
ARDS : is a sudden and progressive from acute respiratory failure in which the alveolar capillary membrane become damaged & permeable to intravascular fluid resulting in sever dyspnea , hypoxemia, infiltrate.

โซ๏ธAs RTs, we should understand
the normal values of our bodies to make good therapeutic decisions.
Down below, you can find a summary of normal values of vital signs, hematology test, and electrolytes

๐บPositive end expiratory pressure (PEEP) is a pressure applied by the ventilator at the end of each breath to ensure that the alveoli are not so prone to collapse. This โrecruitsโ the closed alveoli in the sick lung and improves oxygenation

*FeNO Test used to help diagnose asthma.
Itโs a simple procedure where the Pt. exhales into a mouthpiece connected to a computer.tโ itโs measures the amount of NO present in asthmatic lungs.
Pt. with allergic airway inflammation have higher levels of NO in their exhaled breath.
Pulmonary function testing(PFT)
Indications:
1-Diagnose symptomatic disease
2-the severity for diagnosed lung disease
3-Asymptotic disease in patients who have risk factors
-smoker
-occupational exposure
4-to identity patients at surgical risk
5-monitor response to treatment
๐ตWhat is the Shunt:
perfusion with out ventilation
Shunt can be (capillary or anatomic)
Capillary>when blood flow past unventilated Alveoli
Example:
โขPneumonia
โขPulmonary edema
โขARDS
Anatomic>when blood flow from the right to left heart and completely by pass the lung
๐Hemoptysis
Is coughing up blood from the respiratory tract.
Varies in severity from blood-streaked sputum to frank bleeding.
Massive hemoptysis is defined as 400 mL of blood-tinged sputum in 3 hours or more than 600 mL in 24 hours and is considered a life threatening.

๐ตLaryngeal Mask Airway (LMA)
1.The LMA is designed to be used as an alternative to a face mask for achieving and maintaining the airway during surgery when tracheal intubation is not necessary,or in emergencies when ET intubation cannot be accomplished after several attempts.

PCWP (Pulmonary Capilary Wedge Pressure):
1.Measures left heart function
2.Normal = 5 โ 12 mmHg
3.greater than 18= edema forming in lungs (if no signs CHF think ARDS)
4.greater than 25=edema in lungs from left heart failure (CHF)
5.greater than 12 indicate Mitral valve stenosis.
โ๏ธContinuous Renal Replacement Therapies (CRRT)
are dialysis treatments that are provided as a continuous 24 hour per day therapy.
Blood is removed from the patient, pumped through a dialysis filter and returned to the patient following removal of surplus water and wastes.
What's Auto-PEEP or intrinsic PEEP?
accumulation of air increases alveolar pressure at the end of expiration,it is common with patients on MV
Causes of auto-peep:
1-air trapping(hyperventilation)
2-obstructed airways(decrease EXP.flow)
3-โฌ๏ธEXP.time
4-โฌ๏ธRR
5-โฌ๏ธVt
6-โฌ๏ธTi

Myasthenia gravis:
It is a disorder of neuromuscular transmission resulting from an antibody-mediated,immunologic attack upon the
acetylcholine receptor of the neuromuscular junction.
The hallmark of the disease is fluctuating weakness of ocular, neck, limb, and respiratory muscles
pleural effusion
what is the pleural effusion ??
A number of pleural diseas can cause fluid to accumulate in the pleural space
๐นwhat cause pleural effusion
depend on the cause the excess fluid may be either protein poor (transudative) or protein rich (exudative )
*Evaluation on chest x-ray film *
*Atelectasis*
(1) Appears lighter than normal lung tissue.
(2) May be indicated by elevated diaphragm, mediastinal shift (toward area of atelectasis), or increased density and decreased volume of a lung area.
