Interesting case of the day. Patient with mild degenerative changes in their knee presents with a locked knee after reaching for something in a position of hyperextension. CT imaging performed in ED shows locked osteophytes on the inferior patella and trochlea.

Today I was denied auth by @Cigna to perform a meniscus REPAIR of an acute tear in a young patient. They stated must fail 3 months non-op treatment if knee isn't actively locked. Will they authorize the much larger procedures when the meniscus is no longer repairable?

When an ACL fails over and over again, look for other reasons of failure. Here is a staged anterior closing wedge osteotomy to address excess tibial slope in a 5th time failed ACL. Note the improvement in anterior tibial translation after correction even without a new ACL.

The consequences of untreated medial meniscus root tears are significant. Unfortunately, found another example of their natural history. This patient in their 40s felt a pop while walking up the stairs and in two years has gone on to develop rapid osteoarthritis.

Tunnel osteolysis after ACL allograft reconstruction: When an ACL recon fails, a staged surgical approach is sometimes required. Surgery 1: Remove previous implants and fill the defect of the previous graft with new bone. Surgery 2: New ACL recon after the bone has healed.

Underwent successful closed reduction. However, after experiencing another "locking" episode, proceeded with reduction and arthroscopic excision of the abutting osteophytes. After telling countless patients osteophytes don't cause symptoms, I guess I have been proven wrong.

The popliteus tendon is an important stabilizer to external rotation, internal rotation and varus forces on the knee. Sometimes referred to as the “fifth ligament”, an accurate assessment of the popliteus is crucial in multi-ligament knee injuries.

Increased posterior tibial slope is becoming recognized risk factor for ACL graft failure. The patient demonstrated here had ruptured their ACL for the 5th time. Interestingly, our veterinary colleagues have been treating this in canines for decades with the TPLO procedure.

Success story: Exactly 9 months s/p ACL, LET, medial and lateral meniscus repairs. All PT metrics cleared at PT of Wyoming, and she is back on the slopes! Early in my career, I am not sure if I'm more excited or apprehensive about this video. (reproduced with permission)

While the early literature continues to show good outcomes comparable to other autografts for ACL, this is my major reservation for using the quad tendon in young athletes. This same data was observed by @thetrainingHAUS and still other studies suggest residual quad strength

@mtyrelgarner Take a breath. Self assess. Talk to your family. If you want ortho, biggest advice I can give is don’t be deterred. Happens to great candidates every year. There are plenty of pathways in outside the match, but it’s an uphill battle.

While the PCL generally shields the popliteal artery in medial meniscus repairs, the posterior horn of the lateral meniscus is in closer proximity. This study shows lateral meniscal repairs in the figure 4 position are safest. Repairing from the AM portal can also be a useful

Injuries to the MCL can occur in isolation or in combination with other knee ligament injuries such as the ACL. While the MCL often has potential to heal, stress x-rays may be helpful in determining grade of injury. Gapping of >3.2 mm is associated with a grade III MCL injury.

Interesting study: PRP (3 injections) vs. exercise vs. (PRP + Exercise) for knee OA. Exercise is superior to PRP (no surprises), but adding PRP to exercise does NOT enhance the exercise effect. One to bookmark. We need more of these studies.

Excessive anterior pelvic tilt may contribute to femoroacetabular impingement (FAI). Physical therapy exercises aimed at reducing excessive tilt are beneficial. One recent study suggests a 10 degree reduction in pelvic tilt is equivalent to cam resection:

@DrNickColyvas What do you use to place your fibrin clot in the defect? I've used a smooth cannula and a probe. I've tied the clot to an inside out needle and shuttled it in through a cannula like a zipline. Neither are all that smooth.

@stevemchale In January, I do think repair was still a viable option for this acute tear, especially in this young 45 yo patient. By September/October 21, certainly too much cartilage loss had taken place.

When drilling transtibial tunnels for lateral meniscus root repair, here is one study that highlights strategies to avoid tunnel convergence. I typically drill my transtibial tunnel for the lateral root in the tibial bone block harvest site of my BTB graft.

@DrNickColyvas Once tucked in it works really nicely. I still am trying to do it more efficiently. I use this Arthrex cannula and cut out the plastic leaflets at the aperture with Mayo scissors to make it a “semi-open” cannula. Works ok.

Injuries to the lateral collateral ligament (LCL) of the knee can be challenging to diagnose, especially in chronic injuries as MRI may appear normal. Stress x-rays are extremely helpful in making an accurate diagnosis.

Dr. Krych et al. have shown that lateral meniscus posterior horn/root tears can be encountered as often as 12% of the time in ACL injury. These should be repaired!

Happy Meniscus Monday! The meniscus is a horseshoe shaped structure that acts as a shock absorber to the knee. A radial tear can result in complete loss of meniscus function and lead to rapidly progressive arthritis. Repairing these is recommended when possible.
