After failing to control human #trafficking , #Nepal lays the blame on women and proposes limits to their movement. #Women under 40 must travel with an insurance of $14000 & a recommendation from local officer, unless she's under the 'protection' of family.

@AllenFrancesMD I've seen plenty of people with untreated severe depression in Nepal (because of lack of meds/ECT). They don't "get better naturally"; they become catatonic. I think people in the US haven't seen catatonia and now have a romanticized idea of severe depression.

Meds for BPAD/psychosis in kids if you CAN'T rule it out (vs. only if you can rule it in)? I can't believe they're is a licensed clinician. Hope they get investigated.

Residents/attendings are often in awe of Nepali IMGs. A resident said "he ran the whole floor as an intern. He had run a whole hospital before residency!" @TheUSMLE named Nepal and this has the unintended consequence of unfairly hurting honest Nepali applicants. See/sign below:

@ucsf has -set up temporary COVID screening clinic -made is easy to telecommute and see pts ( @UCSFPsychiatry moved 95%+ of clinic visits online within a week, even if insurance didn't cover it) -canceled elective procedures It's losing a lot of $$ but doing the right things!

As I was writing this, my big fear was in sending any kind of message that we are out of the woods in SF. The takeaway here is that sheltering in place early can help flatten the curve. KEEP STAYING HOME for our frontline medical workers.

Only 37 reports of clots from 17 million people. Not worse than people without vaccines. This is bad timing because many LMICs only have AZ available (unlike these countries that are overreacting by banning AZ but can access other vaccines).

I expected another lawsuit from Nepali IMGs whose scores have NOT been invalidated. @USMLE must act against potential cheating. But there was no need for them to name Nepal in the initial release. They've tainted the credibility of all applicants associated with Nepal.

UPDATE: Last week, a Nepali doctor filed a class action lawsuit against the National Board of Medical Examiners, alleging discrimination based upon national origin and requesting that invalidated USMLE scores be restored while examinees appeal. Today, the NBME responded. (🧵)

If you build, will they come? Yes, but they need to *experience* the benefit. Context: we built a 24-hr delivery center. It was unused for 6 wks, until a woman with post-partum hemorrhage arrived in an emergency, and was saved. After that, there was an influx of deliveries!

You want to help someone quit smoking. Making them feel guilty only works temporarily. So what do you do? Motivational interviewing! But isn't used much in global health. Main reason: it is considered "too Western." We tried to address that. 🧵1/n

Motivation interviewing (MI) has been around since the 1980s and works very well for encouraging patients to engage in healthy behaviors (taking meds, regularly following up with their doctor).

I once had an "Is there a doctor on the plane?" moment on a local flight in Nepal. Crew too scared to let "a paralyzed person" board. Turns out they had catatonia. Convinced the crew it was safe. Explained to family that it was treatable. Family cried in relief. Lorazepam indeed

Lorazepam, lorazepam, rule out medical/WD or need to remove antipsychotics if malignant catatonia or lorazepam not working => ECT and do all this promptly :) and don’t forget the lorazepam and not 0.25mg either. 🙏🏻

@wendyburn @mirandarwolpert @ProfDavidNutt This resonates with what I see in clinical practice. Many SSRIs help people "feel less bad". Psilocybin seems to help people who feel stuck/rigid become more open to newer perspectives/build mental flexibility

Psychiatry is one of the few fields where you can still get to know your patient. My mentor Rohrbaugh @YalePsych used to say "growing up, our family physician inspired me to become a doc. In medschool, I realized it's the psychiatrist who gets to know the patient+their family"

Home visits is one of my favorite things to do with GMH fellows. It's "inefficient" as it can take hours to visit one patient, but you see how they live and learn how tough it is for them to come to see you, and they get to share their story in a space where they're in charge.

I keep seeing "exercise is as effective as meds/therapy" but that conclusion is incorrect. Think about who would agree to 3 hours/week at the gym? Depressed people who agree to an exercise-based study are VERY DIFFERENT from people who choose just meds/therapy.

@AllenFrancesMD When diagnosing someone with BPD, I often bring her book out, go through the diagnostic criteria together with the patient, and if it matches, tell them Linehan's story. Find this to be less stigmatizing for what is a very stigmatized label, and often empowering for patients.

A must-read for anyone interested in mental health. OCD can look and feel like psychosis, leading to misdiagnosis, fear, shame, and incorrect treatments. Every time I've diagnosed OCD, the patient is relieved to get an explanation that helps them go from self-blame to recovery.

Someone with a mental illness is more likely to be a victim rather than a perpetrator of violence. Mental illnesses are universal, but the US has the most mass shootings. Don't use mental illness as a scapegoat for outcomes of racism + poor gun control policies.

I am mentally ill and yet I have never had the desire to target an entire group of people and massacre them. Racism is not a mental illness. It is a choice of hatred and violence. Do not further demonize and stigmatize mental health in order to avoid the dealing with racists.
