sadly in Neurosurgery there are a large number of senior NSurg trainees in the country with no consultant job and the NHS takes advantage of this by giving such jobs they have no option but to take them to better their CV and meet expenses. 1/2

A failed thrombectomy has been playing on my mind recently as to what I could have done better. Today I had a successful thrombectomy and suddenly the sun is shining again. Often it feels as if I’m only as good as my last case. Do others go through similar emotions.

I thought the whole point of PAs was that they’d stick around on wards liberating doctors to go meet their training needs? 2 year top-up course and you get to do all this. If you’re a doctor, good luck with the competition ratios and the exams and portfolio and rotation.

@blondemedSJW Very interesting ppl jumping up and down and saying the radiologist did something criminal by changing CTA to simply a CT in portovenous phase. Actually portal venous phase CT is excellent at picking up most pathologies even ischaemia whereas CTA is useless for other diagnoses.

Let’s clear a misconception here: There are no PAs anywhere close to an Interventional Neuro theatre in the UK. Secondly there is a credential agreed by GMC and royal college allowing doctors from other specialties to train to perform thrombectomy (pls go ahead and train)

@mmamas1973 Wow, the UK/NHS has gone bonkers. You have skills for stroke intervention 100 times greater than any PA. For most ICs, the learning curve will be short. Plus, the cath lab team already knows lots of wires and devices.

Gave some good news to an 84 year old Pakistani lady after doing an ultrasound. As she left she was so happy that she kissed me on the head like adults do to their kids in 🇵🇰. Made my day but also made me miss my parents who are in 🇵🇰.

Who says small aneurysms have low risk of bleeding. One thing is for sure one of these days treating one of these below 3 mm size aneurysms will give me either a ❤️ attack or a stroke just like one such aneurysm nearly did today. Gosh it was a tough one but we made it. 😂😃😱

Today was another proud moment for me. I had placed a femoral sheath for thrombectomy, then my superb trainee @RuiEnChung_INR scrubs in. I let him take over the case and he completed the thrombectomy (single pass) with me just irritating him with funny comments well done 😃.

If someone wants to train to perform a procedure, they need to train properly through an approved system (no shortcuts please we are dealing with patients here). Wanting to expand a service is not a good enough reason to come up with shortcuts to training.

@mlkytee The real issue is it’s safe till something bad happens (then the blameful culture starts). If one escalates now they are considered trouble makers and if you talk after something bad has happened they will say why did you not escalate earlier. 🙈
