What is happening in Arizona ?! My aunt is sick w #COVID19 , paramedics came , oxygen sat low 80s , she's SOB , coughing , fever - they said she's better off at home bc no beds , no guarantee for a bed ...and left an oxygen tank and her PCP started steroids. Self prone.

For my holiday on call, my avg age for COVID hospitalization with intubation, mostly all unvaccinated, yes w co morbid conditions was around 35-40. This is a massive shift from 2020 #covid19 . #GetVaccinatedNow , it's too sad to see - they had a whole life. #COVID19 .

Stuff part 2 1.If you're within 30-60 days post SCT and you haven't engrafted or your graft fails w rash or AMS = HHV6 2. Chromosomally integrated HHV6 will be in the millions 3.Low CMV viremia 1-2k can be a mutation but you're underdosing. IMO we vastly under-dose GCV by CrCL

Stuff: 1. yeast in blood can be trichosporin in the right host 2. Ecthyma gang not always PA can be staph 3. Fever in a profoundly neutropenic patient with all work up neg is OK . Abx switching is NOT ok. Most of the times it's just due to induction chemo and neutropenia itself

I'm a linezolid fan. It's a great drug for certain shorter course circumstances (however have used longer case by case w monitoring), has a wide spectrum of activity, comes in a tablet form. I use it when I can. Thanks for tuning in. #IDtwitter

It's like absolutely impossible for primary teams to stop abx for covid 19 fever. "It's been 3 days" , we broadened him to meropenem, but still having fevers with multifocal pna so broadened him more with vanc Amik. After these 2 yrs we will have no abx left. #COVID19 #IDtwitter

Anyone else feel like LPs are increasingly becoming neuro, neuro-IR, or some other sub specialists problem? I remember doing all LPs at bedside as a resident. Got a consult saying c/f meningitis LP likely won't be done over holiday weekend , need empiric abx coverage. 🤷🏽‍♀️
