This is a fancy way to say the drug gets excreted and then reabsorbed, over and over again.

May explain why some people (poor metabolizers) get minimal pain control.

Iโm not a huge fan of codeine as a pain killer, mainly due to its variable effects and drug metabolism pathways.

Melatonin is generally devoid of significant interactions with other drugs except for ONE exception: fluvoxamine.
Fluvoxamine is a selective serotonin reuptake inhibitor (SSRI) similar to Prozac and Zoloft.
It also is a strong CYP1A2 enzyme

Morphine and codeine tend to have the most patient complaints related to side effects, with the most common one being itching.
The reason? Likely due to the increased histamine release compared to other opioids.
