I’m a nephrologist. The vast majority of dialysis patients who tested positive during the January wave recovered without hospitalization. In fact, hospitalizations for Covid among our vaccinated and boosted dialysis patients were so rare, I don’t even remember a single one.
I feel bad for the people who live in Bismark right now. Because those hospitals are on the verge of being overwhelmed. And this all started a few weeks after Sturgis, like a slowly accelerating runaway freight train.

Yes — it’s worth it because the young are the major vectors for infecting the vulnerable. Most who are distancing are not scared of getting sick. We are scared of infecting and killing someone we love.
Just to finish out this thread, since it’s caught the attention of so many. Once the plane pushed back and engines were on, CO2 levels dropped to around 1500. Combined with HEPA filtration, this makes the cabin air acceptable. But boarding is definitely the high risk time.
Nephrology fellowship applications are down 20% nationwide. We are now over 10 years into this crisis and nothing we as nephrologists have done has worked.

The US had an opportunity to crush disease transmission and squandered it. Boosting does reduce infection rates and prevents breakthrough infections. That evidence is quite clear. By adopting a focus on hospitalizations, we are ensuring an endemic future forever.
Remember CO2 just refers to lack of fresh air return. This can be mitigated by advanced filtration to compensate for the lack of ventilation, but this was not happening on this flight (and I doubt it happens on any flights when they are parked at the gate).

Nearly 1/3 kids admitted with Covid during the Omicron surge required ICU level care. While hospitalizations are rare in kids, they are traumatizing when they happen. Vaccination reduced the risk by at least 50%. Vaccinate your kids, folks.

I find it fascinating how many doctors (including some in PCCM) still view SARS2 as a pulmonary/ARDS virus. It attacks endothelial cells in vascular beds throughout the body. Think infectious vasculitis.

I’ve hesitated talking about this publicly but with the blessing of both Nyan and his family, here we go — Nyan is a fellow in our program (on right) and unfortunately contracted Covid-19 in late May. His symptoms were severe and required hospitalization. He rapidly worsened.
Just received an email from my son’s school about back-to-school Covid protocols. An astonishing 90% of students grades 7-12 are fully vaccinated. Masks will be mandatory for everyone, regardless of vaccine status. This is how you make school as safe as possible.

I think your parents will be okay. Make sure they have a low threshold to seek care if they feel unwell. Ask about availability of Paxlovid. But most importantly, stay positive. Our dialysis population fared very well through Omicron BA.1 when treatments were unavailable.

Not requiring urgent boosting of the entire adult population in the fall was a huge mistake. None of the blue check docs who downplayed the need for boosters in Sept/Oct will ever admit to their complicity in these deaths.
If you had a mild or asymptomatic infection with Omicron, you do not have durable immunity. Without precautions, you can be reinfected within 30 days of the primary infection. This highlights the urgency to develop Omicron boosters and nasal vaccines to bridge the gap.
Name me one speciality that knows fluid and electrolytes (ie, volume status) better than a nephrologist. If you say a cardiologist, it’s an auto-block.

Ventilation was not on. There was minimal air flow from overhead vents, so I doubt there were any HEPA air exchanges during this time. It only got better when the engines were turned on.

I don’t think it’s the responsibility of theme parks to provide masks at this point in the pandemic. However, the fact that Walgreens and CVS don’t carry N95s or KN95s in stores is really problematic. Best bet is to Uber to closest Home Depot and buy a pack of Auras.

I love how you ignore the fact that under 5s don’t have access to vaccine, indoor air quality has been largely ignored, and we still have many patients who are vulnerable that don’t have access to Paxlovid or effective monoclonals. But by all means, Dr. Wen throw away masks.
