Here is a 75 yo severely symptomatic patient with HCM. Resting gradient was 125 mmHg. (Gr 3/6 SEM at LSB) With standing, her LVOT obstruction increased with peak gradient of 225mmHg! (Gr 5/6 SEM at LSB)
