60 yo M with VEXAS syndrome s/p allogeneic BMT was diagnosed with COVID-19. Treated with Paxlovid.
Presented a month later with progressive dyspnea. CT scan below.
Bronchoscopy with BAL was negative. No prior history of GVHD. Not on any new meds.

A patient “left against medical advice”. We called him later and he explained that he was recently imprisoned and feels confined in small rooms. He agreed to return to the hospital. - Good reminder that we should not be judgmental.

A man with diabetes on dapagliflozin has Candida glabrata pyelonephritis. Note the distorted heterogeneous b/l renal enhancement and atrophic right kidney. Sodium-glucose co-transporter 2 inhibitors increase the risk of urinary fungal infection. - How would you treat?
