Looking at the ARV pipeline. The future is injectables. Cabotegravir (cabotegravir and rilpivirine) is now approved for dosing every two months. One shot in each hip every 8 weeks instead of a daily pill. There are better choices coming, now in clinical trials.

I'm sure there are already dozens of cases in the Philippines. They just haven't been diagnosed yet. Currently respiratory cases are a hypothetical risk. We've seen a tiny number of household transmission, but to say how it spread is still conjecture.

For me, sickness means the inability to do your normal daily activities. For me, ARVs allow for a normal, productive life.
I take ARVs daily to prevent illness.
In the same way, I brush my teeth twice a day not because I have cavities, but to prevent them ❤️
For those switching from LTE to TLD: one side effect of the Dolutegravir is gradual weight gain. Also, you no longer have to fast at night. That means more calories before bedtime (which turn to fat). You have two choices: diet and exercise, or pray that the fat goes to your 🍑.
At our weekly staff meeting, I told my boss that I’ll be retiring at the end of the year (I’ll be 62 and can claim early retirement).
This is a big deal for someone who at 32 had a viral load over 1 million and was expect to live for only 2 more years
When we become HIV+, we still have other things going on in our lives.
We still have to go to work or school, juggle our finances, work towards our goals. We still get horny and hungry and lonely.
Now in addition, we have to manage our medications and go in for lab work.

Science has achieved what seems close to the impossible. They have created a single pill that stops HIV in its tracks and turned a fatal disease into one that's easily controlled. You can't say the same for diabetes, respiratory diseases, or hundreds of other chronic conditions

Unfortunately, tests used to screen the blood supply have a window period where there could be a false negative. As screening technology improves, becoming both more sensitive, rules will change. Remember that donations are pooled, so 1 unit has the potential to infect hundreds

We had lots of clients in Montecito and Hope Ranch (we did flowers for Julia Child's Dinner with Julia) who had standing weekly or monthly orders, and they would get a monthly bill. We had a bookkeeper who sent out bills and there'd be no interest charged if someone's didn't pay.
After six decades on earth (and living with HIV for half of that), the most practical thing I’ve learned is the power of the present moment. The past is the invisible pit that pulls us under. The future is unknowable. It doesn’t yet exist (and it never will).

Let’s talk about HIV disclosure and dating. For 12 years, I’ve run a newly diagnosed support group and had hundreds of conversations on the topic.
There are no wrong or right answers and we each have to follow a path that’s comfortable for us.
First, let me share some options

We have the tools to eliminate HIV transmission: HIV testing, biomedical prevetion (PrEP), and ARVs that make transmission impossible.
These tools are being implemented worldwide, and the number of new infections is going down.

Here's an interesting and small study, with 24 participants. It compared 2 popular ARVs in North America and Europe, Biktarvy and Dovato. These were all newly diagnosed blood brothers who were followed for six months. Viral levels were measured in blood, semen, and rectal fluid

We've had 279 cases in LA County. In my clinic, 5 all sores were on the penis, perineum, and around the anus and mouth. One had pustules inside the rectum. Better advice would be to dial back on the multiple partners thing until you get vaccinated.
I see my HIV doc for 15 minutes, 2X a year. That includes an exam, going over labs, and any questions or issues I have.

A big barrier to an HIV cure are resting T-cells infected with HIV. They don't cause harm, because the cells are dormant, therefore the HIV can't replicate. When some cells activate, so does their HIV, which is why we have to continue to take our 💊

Overall, drug resistance is on the decline. I think in the early days it was much more common. We were prescribed single- then dual-drug ARVs that would work for a short while. Because the virus wasn’t totally suppressed, HIV could mutate and resistance would emerge.

About ten years ago, I got a grant so our hospital’s ER could offer opt-out HIV testing. That means everyone who got a blood draw in our ER got an HIV test, unless they “opted out”. We went from 1-2 tests a month to 1,000
Our CD4 numbers change from morning to night and will fluctuate depending on the amount of sleep we had the night before our blood draw (among other factors). A better number to track is our CD4%, which is more consistent.
After a few years, our CD4s will level off at our
