Proud to have presented at the successful Inaugural Paediatric Heamatooncology workshop this week, discussing paediatric brain tumours.Organised by the Kenyatta National and the amazing Paediatricians and Haematooncologists Irene Nzamu, Yvonne Mbithi,Cathy Mwendo amongst others.
Congratulations to all the post graduates and undergraduates. Now you get to decide what type of Drs you get to be(serious Dr, funny Dr, strict Dr ) regardless of you choice the only important one is Good Dr. As long as you keep trying to do that, you’ll be great. Be blessed all.
We are grateful for those who have come before and those who continue to steward, good or bad we know that, we are only here because those before did not give up.
To all the medics working over the holidays, take heart and remember why you are doing this, to give others a chance with their family members. Be blessed all. Happy holidays.
They may not be much, but at least they are mine. If any of these help even one person then I am blessed. You can complain about any situation or you can try and change it.(The latter is always more fun.)
Brain tumours are some of the most common solid tissue tumours in children and headache is usually one of the earliest symptoms. Here is some of what to look for.
Fun fact the average publication costs 35 USD to get access to, what happens when you have to review 15 on a singular subject? That’s where these sites come in.
You cannot mandate productivity without providing tools for people to be their best. A tool is no replacement for a craftsman. Here are some useful ones.
Good luck to the finalists starting exams tomorrow. Luck is when preparation meets opportunity and you have prepared for the past 6+ years. Be blessed.
Cranial demoid cysts are some of the most common paediatric skull tumours. They arise from entrapment of ectodermal elements along lines of embryonic closure.
If you are considering getting pregnant, start folic acid supplementation prior to pregnancy to avoid Neural tube defects like Spina Bifida. Click on link for full video.

The midline suboccipital approach can be used to access vascular, neoplastic, and developmental pathologies of the cerebellar hemispheres, vermis, fourth ventricle, brainstem, and foramen magnum.
No clinician can work alone, focusing on building effective healthcare teams, definition of health care errors and establishing team outcomes goes a long way in decreasing medical errors (it doesn't matter how good you are if your team is useless).
The 12 Cranial nerves are delicate structures susceptible to damage by mechanical trauma or ischemia during intracranial and extracranial surgery. In selected cases we monitor them to preserve function and prevent injury to them when clinical examination is not possible.
Spina bifida is an easily preventable birth defect in which the spinal column fails to form during fetal development. Unfortunately this tends to happen early when mothers are unaware they are pregnant.Peripartum folic acid supplementation is crucial in its prevention.
Have you heard of the pituitary gland? A tiny gland at the base of the brain responsible for regulating a lot of the body's hormonal functions including growth, metabolism and reproduction? This is how it is formed.

Studying of images and knowledge of surgical anatomy is vital for good post operative outcomes. Every good surgeon does the surgery mentally multiple times prior to cutting the patient.
Sylvian fissure dissection allows exposure of the structures around the anterior circle of Willis, parainsular regions, mesiobasal temporal lobe, and interpeduncular cisterns, with minimal retraction of the normal brain parenchyma.
Clonus is a series of involuntary, rhythmic, muscular contractions, and relaxations. It may be caused by interruption of the upper motor neuron fibers such as by trauma, stroke, multiple sclerosis, or by metabolic alterations such as severe hepatic failure or serotonin syndrome.
Cortical mapping is an invasive procedure in which electrical stimulation is applied briefly to the cortical surface for the purpose of identifying areas critical for sensory, motor or language function, during surgery of eloquent areas.
Although it might be comforting to think that there are different types of intelligences, the good news is “Intelligence” is not distinct entities but a broader ability to be able to learn multiple things I.e: You can be good at math, science and good at art.(You are limitless).
Caseating tuberculous granulomas are well defined focal masses caused by Mycobacterium tuberculosis(TB),this is one of the most severe morphological forms of TB. In 2019 TB was reported as the 5th leading cause of death in Kenya by the Ministry of health.
When loss of motor function is of significant concern in either cranial or spinal surgery. Intra-operative TCMEP monitoring is a significantly reliable method for preventing postoperative motor palsy in both cranial and spinal surgery.
Laminoplasty which is widely used spinal surgery, esp in cases of intradural tumour and lumbar spinal canal stenosis (LSCS),can preserve the posterior arch of the spine, protect paraspinal muscles, and prevent postoperative instability, epidural adhesion and kyphotic deformities.
"May is Brain Tumour Awareness Month, and I want to shed some light on craniopharyngiomas - rare brain tumours that can occur in both children and adults. These tumours develop near the pituitary gland and can affect vision, growth, hormone levels, and more. Let's spread awareness
“The workhorse”.The characteristics of this approach in its current form include drilling the sphenoid wing as far down as the superior orbital fissure with or without removal of the anterior clinoid, thinning the orbital roof, and opening the Sylvian fissure and basal cisterns.
Regardless of how straightforward a surgical case is. Minimisation of blood loss via good haemostatic techniques is a core tenet of paediatric neurosurgery.
The trans-sulcal dissection is a safe and effective approach. Through which it is possible to reach deeply seated lesions. It preserves the motor functions, provides a larger field of view, minimizes the need of brain retraction during surgery, and keeps the gyral layers intact.
The Sylvian fissure Is the fissure that divides the temporal lobe from the parietal and frontal lobes.
The ability to dissect Sylvian fissures is crucial in microneurosurgery. A favorable prognosis requires the sylvian fissure to be opened safely and precisely.
What does tumour surgery without a scalpel or a even a wound look like? It’s called Cyberknife(Image guided robotic radiosurgery). Click link for full video

How do you preserve sensation during surgery? The identification of Rolandic (eloquent) cortex with the use of intra-operative SSEP monitoring enables the neurosurgeon to tailor the surgery to achieve a greater extent of resection while minimizing the risk of morbidity.
See you at the 5th Annual conference of the Society of Neuro-oncology Sub Saharan Africa Happening in Nairobi!! Open to all interested in the management of brain tumours!!
Anterior cervical discectomy and fusion (ACDF) is a surgery to remove a herniated or degenerative disc in the neck. An incision is made in the throat area to reach and remove the disc. A graft is inserted to fuse together the bones above and below the disc.
(TLDR: You can learn anything at any age) Neuroplasticity, the brain’s extraordinary ability to rewire and adapt, is the secret sauce behind our remarkable learning journeys.
Let’s break it down and discover how it propels us to new heights!

(TLDR: Forgiveness is literally good for you)Did you know that forgiveness which is a cognitive and emotional process that is a function of the dorsolateral prefrontal cortex (part of the frontal lobe) is directly responsible for a net positive health effect.
Posterior communicating artery aneurysms are a common site of aneurysms, accounting for up to 45.9% of all aneurysms(in multiple series), and have a high rupture rate. Oculomotor nerve palsy is a common clinical manifestation due to their adjacent anatomical relationship.
There are over 150 types of brain tumours. Oligodendrogliomas are a type of glioma that are believed to originate from the oligodendrocytes of the brain or from a glial precursor cell. Full video on YouTube here
The anterior communicating artery (Acom)aneurysm is one of the most complex of all cerebral aneurysms. The subfrontal approach provides an efficient avenue to the AComm region, which reduces opening and closing friction and still yields a comprehensive operative window for access.
Professional advocacy on prevention and awareness of Spina bifida. Will go a long way in preventing this defect and hopefully inspire multisectorial efforts in helping those affected by the illness in Kenya.
