We all mourn the days before covid when we thought it was okay to not mask for the majority of our in-person patient interactions. It felt great.
Now we know so much disease is airborne and we can easily be vectors.

This is my third day of having COVID. We did everything right: vaccines, ventilation, HEPA filters, limiting indoor public activity, etc.
I got covid because it has been allowed to run rampant in our society.
This is a deadly and disabling disease.

Patient's family member yesterday, "But if covid is still so bad, why isn't the government doing more to educate us?"
"I don't know. What I *do* know is covid appears to be damaging immune systems, and increases risk for heart attacks, stokes, dementia and many more things.

We have a deadly and disabling airborne disease just being allowed to circulate.
I now have a fever, cough, etc. I am sick. I care for patients. I am getting PCR testing, but we have to cancel/reschedule my patients booked for tomorrow and likely next week.

The reason most people aren't taking covid seriously is because they simply can't imagine Public Health would abandon protections and let an incredibly disabling and airborne virus spread wildly.
They will be very angry when it becomes clear to them that's exactly what happened.
Heads-up: Long-COVID affects 25% of kids who get covid. The main problems: mood symptoms, fatigue and sleep disorders.
Covid is a mass-disabling event and we are injuring the children in our society by not working to stop its spread.

Patient today:

All great questions. All without any defensible answers.
Today I had a patient say, "But I thought COVID was over?!?" A smart person, reads the news, etc.
He was stunned to hear of long-covid and post-covid increased disease is overwhelming us. And put on a mask right away, making sure it was snug around his nose.

This paper is terrifying.

I'm an experienced family doc. If you're sick, you're sick. Just because zoom and other virtual technologies exist, you should not be working when you're sick. Please take care of yourself first and get better. If your goal is maximal productivity, that's how to get there. 1/
It will continue to spread, infect more people, kill and disable more people, and cause health system and economic collapse.

I'll be seeing a patient this afternoon outside in his car. Why? Because he has covid and neither he nor I want to expose other patients to it. I don't think this happens very often, which is just one more reason why we need a REAL mask mandate for ALL clinical spaces/bldgs.
As a doc, I will say clearly: indoor spaces need to be examined by professional engineers so that they can deploy air quality control systems.
Prof engineers and occupational hygienists are the ones who need to be enabled to end this disabling pandemic.

These are average weekly excess deaths in Canada by year.
Weekly excess mortality in 2023 is still higher than 2021 or 2022. We need to take our heads out of the sand NOW.
"Patient cancelled their appointment," is a trigger to give them a call. There is often a medical or medically-adjacent reason for the cancellation.
My most recent scenario? The patient had an acute viral infection. I coached how to do Rapid Test properly over the phone

Cancelling a medical appointment is an incredibly difficult decision - we donโt make it lightly. It doesnโt mean our health isnโt a priority.
Often we canโt physically GET to appt. I passed out on floor of apartment hallway trying to make an appt once. Still said I โmissedโ it.
Trans people just want to be left alone, be able to make medical decisions with their doctors without politicians, and be able to use their preferred names and pronouns with people they trust.
They deserve the same right to life as everyone else.

Foothills Emergency Dept where I have experienced + witnessed great patient care. It's the same ER where my sister had 6 cardiac arrests from a pulmonary embolus and later died in the ICU in 2011. My family doc and the ER team just saved my life. 1/
It's not normal for every emergency dept. and urgent care centre in the city to have wait times from over 5.5 to over 12 hours.

Thanks for the quote tweet. I now know of 4 docs who are on long-term disability due to long-covid. We are disabling (and even killing) our healthcare workforce exactly when we need them most. All we need are mask mandates, clean air, and other sensible measures to protect us.
A child's right to life takes priority over a parent's right to know if their kid is using different pronouns.

They're hot and uncomfortable, even with the most breathable ones.

I'm at a specialist appointment right now. Almost no one else is masked.
But, amazingly, there are two other patients who are wearing well-fitting respirators! I thanked them for doing so.
I just glanced around again and there are 2 others wearing basic masks too.
It is never too late to start taking cov1d seriously again. It still kills. It still causes disability and chronic conditions (around 10% chance EACH time you get infected, 5% if vaxxed in past 6 months or so).
So mask up, get vaxxed, test frequently, isolate if symptoms.
