I am sharing my lipid profile. Left 1/2020; Right 4/2021.
Despite my COVID-19 weight gain, my lipids are at goal. My CAC is 0 but I have read too many CT scans to want CAD to develop. Thx to baby daily statin!
This is primordial prevention! Hoping to be CAD-free when I am 80!!!
I am being redeployed.
Full of mixed emotions. I want to help but scared. Scared of getting COVID and that I am NOT as good as my younger self during the AIDS crisis, 9/11, or the Boston bombing.
I know I can do this. Someone has to be on the

Don’t take it as a negative. Take it as a positive that your busy attending cares enough to help you get the best message out. He/she could easily just let you submit with minimal edits, and get your work rejected by much more critical reviewers.

It’s egregious. My high risk parents are in OC, CA. There is only Disneyland that vax age>65. Took my brother a week to get through online to make appt, only to find out line at Disney is 2-3 hrs long w/ appt. Parents are old 73 & 83, & can’t do that. So they have not signed up.

Why are we not modifying the vaccine to be effective against Omicron? Is it really necessary to booster with a 4th jab with old vax that is not effective against Omicron? We need to be faster in updating our vax strategy. We don’t give the same flu shot every year.

What is your baseline T cell levels? Are your T cells low and thus you are prone to more infections? Or the converse, you are get infected and thus T cells low? Without having a baseline level hard to interpret this result.
@JACCJournals
Answer: Single coronary with anomalous LM arising from the RCA with a transseptal/subpulmonic course and a long intramyocardial segment.
Given recurrent CP, pt underwent external unroofing.
Avoid CABG alone in pts w/ good forward flow due to high likelihood of graft failure.

Let’s clarify a couple things about
#Hydroxychloroquine
Hydroxychloroquine has a pretty benign profile and does NOT need baseline ECG.
#HydroxychloroquineAndAzithromycin
combo can prolong the QTc and may cause
#torsades
in pts with baseline prolonged QTc. ECG should be done.

Answer:
1️⃣Membranous/perimembranous ventricular septal defect (VSD)
2️⃣Patent foramen ovale (PFO)
3️⃣S-shaped sinoatrial (SA) nodal artery from distal LCx. Caution should be used if left pulmonary veins are accessed (eg ablation).
CT nerds-not fistula-SVC, it is SA nodal region.

Bonus question: how would you treat or manage this?
Hint: Interventional folks should be able to read this also!
@venkmurthy
Non-RCT prospective without control is a prospective observation study. Case series are retrospective and small sample size and without a comparative arm. I don’t consider retrospective or prospective observational studies as trials; they are studies. Trials are RCT.

Obviously taken out of context. That’s why I decline interviews with reporters.
1. You don’t get paid for it.
2. You won’t be famous for it.
3. It may backfire and they may take things out of context for a juicer storyline.
@venkmurthy
This is ridiculous. Talk about micro-aggression and micro-management. Smoker is shorter and simpler to use. All this rhetoric needs to stop.
@Toaster_Pastry
there is no such thing as cardiac clearance. there is risk stratification. pt is high MACE risk for surgery. given emergent nature, no further testing is needed prior to surgery. avoid hypotension. cardiac anesthesiology would be helpful.
60 F with hypertension, hyperlidemia and dyspnea. MD ordered exercise stress test + calcium score. 1 month later, CAC=0 and positive ETT. Coronary CTA order.
2 months later-pt will have gotten 3 tests…could have gotten answer already if coronary CTA was ordered first. 🤦🏻♀️
