Working here on a Sunday is the highlight of my week ☺️ We are a cafe for those who are vulnerable in swansea. Everyone gets music, company a free 2 course meal, multiple hot and cold drinks, dignity packs and groceries/treats to take away. We also have showers and a Laundry

10yrs ago I had an offer to study medicine. I didn’t meet it. 4yrs loving someone who broke my bones & told me I was stupid instead. I became an ICU nurse (1st BSc hons). I’ve just finished my 1st yr of medicine. Life happens. But there’s always a way. #resultsday

Nearly broke a bleep on purpose today, when for the Xth week in a row the 2222 announcer yelled out “HOSPITAL OPERATING AT RISK LEVEL PLEASE CAN ALL TEAMS REVIEW THEIR PATIENTS” Well, bugger me. What on earth do they think we do every day? Fewmin. it’s. Just. So. Insulting!

Cramming a medical degree into 4 years is enough thanks. Can't imagine what they'd have to remove to get it to 3 years. I thought being an RN first would stand me in good STEAD. In some ways it has. But OH MY GOD THERE'S SO MUCH MORE TO LEARN I DIDN'T EVEN KNOW EXISTED.

👏🏻👏🏻👏🏻👏🏻 FINALLY VINDICATION. I swear, the amount of side eye i get for not wearing gloves when theres NO NEED TO. Gloves sit in an open box being exposed to everything around for DAYS 🤢(poo particles) gloves on freshly washed hands defeats the point unless risk of BBV.

This is reassuring to me... for a while, I’ve been a bit of an outlier on the team when taking blood or putting in a venflon, that for the vast majority of patients, I don’t wear gloves. Article from @bmj_latest .

2nd jab done. Fully vaccinated. To think that less than a year ago, COVID19 hadn’t really hit the UK yet....we had a handful, if that, of cases. Scientists, you knocked it out of the park guys. Forever grateful.

So despite being informed I have an interview and being booked for one as per email - it turns out I do not have an interview and just been emailed by ANRO to say they it was an error and I, infact *do not* have an interview. So that’s pretty devastating.

Sadly, my uncle passed away in ITU yesterday due to corona. Much too young. True gent and an awesome pub landlord. Feeling so awful for my isolated nan who has now lost two sons. RIP our Tone. Bevs on you fella 🍻 x

Love finding out 5 days before I start that I have no accommodation&they’ve never heard of me because the person dealing with my request (from *months* ago) is off. NHS: Enjoy your new career! Also, you’re homeless (thankfully, I have other back up places to stay temporarily)

So the other half (who doesn’t even have a GP because he’s never Ill) young, fit, no PMH: can’t walk up a flight of stairs without needing to stop and catch his breath and is coughing +++. Negative PCR. I remain suspicious.

@clare_eliza I’ve been trying my best to get people out of A&E and under us but there’s just no space to see anyone. I’ve resorted to taking them to other parts of the hospital to take bloods/catheterise etc I’ve asked the seniors if we can set up a temp area on the ward, I suspect the (1/2)

@DrLindaDykes Any sick patient who asks for the commode/bedpan to have a poo. I don't care how stable their last gas was or their ECG currently is..... You should always hover V. close by and be prepared to pull the emergency buzzer.

Oh my word. The power of twitter. 24hours ago the hospice tweeted a plea to see the new star wars film (not yet released) as its unpikely the patient will make it that far. Disney CEO responded and 24hours later, hes seen the film with his son. Amazing. ❤️

It’s ok, I didn’t meet the cut off - fair enough - so I don’t expect it to change. I’ll clinical fellow for a year, do better in the MSRA and try again next year (and console myself in the meantime with my excellent ICU feedback!) But it shouldn’t happen to anyone else x

Just had a job interview for F3 year with what I presume is a dental infection and oh my days, how I didn’t manage to claw my own tooth out on camera mid-interview, is beyond me. Can’t wait for the emergency dentist to be liberal with the lidocaine at 13:50. 😢

What is this obsession with making everyone turn their cameras? Sure, it makes you feel a bit less ridiculous as the presenter/teacher/etc But, not everyone has a suitable or good homelife that they want broadcast to everyone...would have thought it an obvious consideration. 🤔

I really really love my GP placement. I still don't think I'd be a convert long term but it's so fantastic. This is the difference a good team, good practice, GPs that like teaching (and amazing teaching at that) makes. I certainly wouldn't be unhappy to work here.

First day of anaesthetics today! Huge thanks to my current ICU dept where I’m working as a clinical fellow, allowing me the time to develop and investing me as a doctor. I was so sad not to get a training number…..but I am having *the best* F3 year.

OVER THE MOON WITH MY RESULTS... it was the worst exam period I've ever had. I've never had exams completely throw me like these did. I'm still rubbish at anatomy 😢. But diagnosis and clinical skills are doing well 😁 After weeks of holding my breath, I can finally breathe

Two straight weeks of ITU and corona assessment unit nursing done ✅ 6 days off now, back to Med Student mode. 🩺📚 Ignoring all the “PLEASE COME TO WORK” pleas via text. Got some learning and a whole load of rest and recharging to do. Slowly learning how to juggle both.

As an ex dancer, I sometimes struggle with strictly in the early weeks (even though I love it!)… …but for the first time, I was moved to tears by a dancer tonight. Rose, you are exceptional and that was incredible. To fill the empty spaces in music is a skill. #Strictly2021

I doubt there’ll be an anaesthetics interview for me this year *however*, I get to continue in my clinical fellow role in anaesthesia/intensive care…continue all the projects I have going on, get the IAC as well as continue setting up my home. So i’m happy, roll on next year!

tldr; medicine is bloody huge. I don't care if you're the Stephen hawking of nursing.... You *do not* want to attempt a medical degree in 3 years. *possibly doable if you don't mind sacrificing every holiday, your mental health & can pay 3 years worth of living expenses upfront.

@clare_eliza (2/2) sticking point is it would need an extra nurse. Just doesn’t sit right with me that they’re accepted under us, but still an A&E problem if they become acutely unwell in the WR while awaiting a bed. Also lack of A&E space slows us down in seeing referrals etc…

Had a realisation: in medicine *literally* every day you get paid to solve problems, ask questions, think about things, do practical things all with the end goal (if not outcome) of making people feel better. Thats pretty special really. Even if, at times it can feel very “meh”.

I’d just like to point out too, that the same applies for ACCPs….they have been enormously helpful to me starting in ICU as a doctor and when shit hits the fan, I’m always pleased when one of them arrives. They’re knowledgeable, dependable, constant and very, very capable.

And, my advanced care nurse and paramedic colleagues must not be taken into this debate. They have years of experience before taking advanced roles and are a totally different cohort. They have not had a two year course and told "they work just like Doctors"

Good news on the NHS just out: There are 13,313 more nurses & 6,030 more doctors in the NHS than a year ago We're well on track to meet our manifesto commitment of 50,000 more nurses over this Parliament

Having flashbacks to OSCE finals when I couldn’t remember the word “adenosine” and just panicked and said “BETA BLOCKERS!” for SVT... I use this drug *all the time* in the day/night job. Would the word come out? Nah pal, would it feck.

ITU as a med student this morning. I take back all my grumbles....... so alot of it I knew, but I got pushed today making plans on the ward round and it was soooooooo good, learnt loads extra! Lots of little links in my head finally clicked into place!

This year, to summarise: I have completed all placements, passed all assignments (albeit with an extension in one due to the world imploding), worked fulltime as an ITU nurse during a pandemic and survived, got diagnosed with ADHD, nearly paid off all my debt (1)

Jayzus. >2 hrs of pH 6.6, PCO2 13-66kpa with full neurological recovery. This is a rollercoaster (and quite the testimony to physiology and anaesthetists everywhere) from 1997. Thanks to @JohnRossMD and @thomasngmorris
