Thank you for highlighting a sore truth, we as IMGs got here today because of the altruism of many other IMGs and US physicians. To use the vulnerability of an IMG and charge them is a horrible new trend. Same goes for charging humungous amounts for very poor
People who extort IMGs by making them pay to review their PS are the worst. Also just because you matched doesn’t make you an expert in the match and how to write personal statements.

Ephedra is a Chinese herb that has active ingredients ephedrine and pseudoephedrine, this was banned in US abased on a meta-analysis of 52 studies (Shekelle et al. JAMA 2003) it increases risk of MI (heart attack), seizures and psychiatric illness in young people, dose 60-120 mg
The FDA and European Union banned dietary supplements containing ephedrine because of their serious safety risks.
Especially in connection with cardiovascular toxicities including myocarditis, arrhythmias, myocardial infarction, cardiac arrest, and sudden death.

1) Set goals and expectations day 1 of your rotation make 2-3 goals for yourself to get out of this block, also ask your staff what are their expectations from you. Every staff expects different things so getting a head starts allows you to set yourself for maximising learning
3) Leading your team, fellowship is about learning to lead, while as a resident we get good at doing things, now we need to learn to supervise. Hard transition but a meaningful one

I absolutely agree that this is borderline exploitation (borderline since it isn’t illegal). It up-trended in the pandemic. There needs to be more open debate about it. While it may allow for career growth, the upfront cost in this time of inflation might just be too much…..
Unpaid research fellowships for IMGs should be illegal, working for 1-2 years without getting a salary on the off chance of matching and starting residency is borderline exploitation
