@IM_Crit_
My first question to family is “tell me what you understand is happening with your [relative]” and usually it is like “no one has told us anything…the nurse said they were moving him here”. Usually 10-15 minutes later drips are off, DNR completed, comfort care transitioning.

A high no show rate reflects negatively on the administrator as it is their job to ensure the schedule is filled and to keep it that way. They may need to hire more docs, leverage tech, assist provider to design schedule templates that work, etc.

Did they explore for a tumor or do any additional testing post-op to determine why she was hypertensive and had abdominal enlargement causing her to perhaps believe she was pregnant?

The programs should set the tone and simply pronounce to the teams all the students are off for all holidays. For Thanksgiving, just take off the four days. Everyone else divides the work and gets out as fast as possible. The students are paying you—not the other way around.
@USDOT
What is or is not within their control should require independent 3rd party verification. Too many excuses for weather when other airlines seem to be able to fly. Since taxpayers have to keep bailing these airlines out, we deserve more accountability and reliability.
@AutismCapital
Hmm…maybe this is why FTX didn’t have a board, Caroline had opined “do you really need a board comprised of a bunch of guys who are friends of the CEO”? 🤔 There really needs to be a whole documentary dedicated to ‘Caroline Ellison’.
@drcadesky
Curious how much it costs. An MRI brain can be $5K alone in hospital-based facility in the US. Have had several patients from India who take their scrip from me there when they visit family and get same scan, on the same brand machine--for $200! I get report in English.
@jtrebach
Consider heterotaxy syndrome (situs ambiguous). Would be key to see pre-operative imaging and radiology reports. Still does not justify extirpation of the liver instead of the spleen.
@RammahaSahar
Diabolical. I let students do this all the time. They had an exam, family in town, sick family member…I actually did not care as it is there rotation/paying to be there. They got other things on their mind or life goes on…take care of it. Students are there to learn, not earn.
@jbcarmody
Great 🧵! I completed residencies before and after this timeframe. The duty hours changed but PGY salaries remained the same and have changed little since. Never knew the back story. Thanks for sharing!

There should be compensation for all the time spent with, or on the behalf of, a patient. Here is a screenshot of an attorney’s monitor where you see they bill in these increments:

They want to claim it was out of their control, per their customer service agreement. What other business can take so many people’s money and simply fail to deliver and implore them to “go home”. I get weather delays when it legit. But half the flights for the day? 🤦🏼♂️
@VipsMDMEd
My favorite is the consult from a hospitalist triaging admissions who themselves hasn’t seen the patient but places an order for an ED admit that was turned over to one ED Doc by another who never saw the patient either and there are no notes at all in the chart.
@unusual_whales
Absolutely. It would be insane not to jump into this, like the March-April 2020 dip. You don't get this lucky with this degree of generational wealth-building dips more than a few times in your life, like we have the past ~ two years.

No, if they are in a high-end car I wonder how much I am going to get taken for. Save that for your own time and show up in a clean 2-3 year old SUV.

The Founding Fathers of the USA felt that citizens should be able to protect themselves against the government and any other threat to their wellbeing or personal freedom. The Second Amendment granted citizens that right — giving them the ability to defend themselves + property.

I completed two residencies: 1st emergency medicine, before the 80-hour mandate, 2nd neurology, after—I was so much more prepared in the former. I love medicine and when I walk out of room after telling a family their loved one will talk and go home after a massive stroke—🤙🏼

What pharmacists and physicians and other healthcare providers need is two-way communication within the EMR and these seeming annoyances would be tools to safer and more timely care for the “team” caring for the patient. Phone calls in 2022 for this…🤦🏼♂️

FTX (the FTX US notwithstanding) was incorporated in the Bahamas. The companies don’t have to comply with any annual reporting requirements and are not subject to any exchange control laws.
They are allowed to transact freely in any currency. So how is this going to work?

In psychiatry this is termed “grandiose delusions”. GDs are characterized by fantastical beliefs that one is famous, omnipotent, wealthy, or otherwise very powerful.
@unusual_whales
If you lived through the last two years and see how beaten down small businesses and retail are, then you might predict with the costs of goods and services at record levels, it is only a matter of time before something has to give.

The left is a full metal jacket (FMJ) bullet wound and the right is a shrapnel wound such as may be seen with an IED or anti-personnel artillery munition. FMJ is almost exclusively used for target shooting or military ops. Jacketed hollow point rounds may cavitate.
@JakeGoodmanMD
Either you get the training or the patients die. When I was chief in neurology, the noon hour came and those post-call went home and I did all the LPs, a-lines-, central lines, etc myself after rounds because they had to leave.
@CSWhiteMD
The length of the wait predicts the magnitude of the no show effect. Time to third next appointment metric helps to know when to hire more docs/APPs, grow the practice, leverage telehealth. Overbook no show rate to limited extent. Increase more same day slots and so on.
@unusual_whales
Yes, we are in the recession already. Does anyone really think that with the costs of goods being what they are, that businesses will be able to maintain the status quo without cutting costs—this is what happens in a recession. Revenue falls, costs are cut.
