I spent many hours last week observing the practice of medicine while sitting at my mom’s hospital bedside and was reminded of some important communication pearls. Some musings…

"Please tell my wife I love her."
Parting words from my patient as we left his hospital room. We have a no visitor policy (again).
My eyes welled. I was reminded of my dad as he died alone in a nursing facility of COVID. It is *every* patient right now.
Please, wear a mask.
Today, we lost my father, Tom Kunz, to complications of
#COVID19
. He was an incredible husband, father, and papa. He cared deeply about many things - his family, his profession as a biologist, and the environment.

8. Talk with your pts about something other than what brought them to the hospital (a tip I once learned from a wise mentor

). Patients are more than their disease.

1. Introduce yourself by full name, role and team and have ID badges visible. It can get very confusing for pts and family members with the number of people in and out of rooms.
e.g. "My name is Dr. X. I'm the intern on the primary internal medicine team.”

2. End your patient visit with a summary of the plan for the day.
3. Avoid medical jargon & speak slowly, clearly, and logically. Remember you are a teacher for your pts and their family.

7. Try to avoid last minute or surprise discharges – they make pts and family members anxious. Talk about discharge planning from day 1 and what milestones must occur prior to a safe discharge.
“In order for you to leave the hospital, X, Y, X must happen.”

4. Masks make it harder to hear especially for pts with hearing loss (and they no longer have the aid of lip reading).
5. Many older pts get confused in the hospital. Repetition is a good thing.
6. Speak to a family member at least once per day to relay the plan.

Today it got real. I received the call I've been dreading. My 82 yr old, chronically ill father who lives in a SNF is now
#COVID19
positive. I'm sad and scared. He's alone, no visitors, no phone. Had hard conversations with mom and brother. Please

On this Thanksgiving Eve, I am sharing a deeply personal experience in hopes that it will change some behavior before the upcoming holidays. My father died of
#COVID
in April during the height of the last peak.

1/ After 19 years at Stanford, Friday was my last day. The pandemic goodbyes were different, but my awesome team made it perfect and meaningful (with a special farewell video montage). I have many thank yous and some parting thoughts…💔😢
It's been a hard week. I've had to play daughter instead of doctor while my mom has been in the hospital.
I cancelled most of my clinic tomorrow and called some patients tonight to explain.
One of them flipped the script and said "Take care of yourself and eat a pastry!" 💕😢
As for me, I will continue to be a mentor, ally and sponsor to
#WIM
and will contribute to objective data on gender disparities in medicine. How will YOU inspire change?

I just got off the phone to arrange a new service in my home.
After giving me the bid, the scheduler asked "do you need to check with your husband?"
No, no I do not. I can make a decision to spend money independently. Unconscious bias all around...😞
Celebrating a small win today...
A handyman working at our house asked me about the safety of the COVID vaccine. He is age-eligible in our state. We talked about the science and the risks/benefits.
Him: "Thanks, doc...I'm going to get it now."
Science for the win.🧪💉💗
He also lifted many others. He was a dedicated mentor for young scientists and was proud of their accomplishments. His research focused on the behavior, evolution, and biology of bats. To many, he was fondly known as “Batman.”
We will miss him.💔

As I entered mid-career, assumed leadership roles, and gained some degree of success, I was perceived as a threat by some male colleagues. At that point, I experienced insidious microaggressions and retaliation from male faculty.

It is my great honor to be the incoming President for

. In this role I will support the society's mission to educate our medical community and promote innovative science - both of which will lead to better patient care. 💕🦓
These ranged from public put-downs and disparaging comments to exclusion from research collaborations. I felt alone, demoralized, and stripped of my self-confidence. The microaggressions and inaction when I raised concerns led to a toxic environment that I couldn't tolerate.

I'm sad for the victims and deeply inspired by their bravery.
I'm sad for the profession of medicine that this behavior is tolerated and enabled in 2021.

Feeling nostalgic this week. It's been 2 yrs since my dad's death. We took the kids back to my childhood home where Papa built them a treehouse. So grateful to the current owners for allowing us to make these memories. 12 yrs between these photos!

Stanford was a wonderful place to train and start my career. I had exceptional mentors and colleagues and was given support to start a neuroendocrine tumor program. But then things changed.

Reading "The Night Before Christmas" is a long-held family tradition. My dad loved to read this book to me and my brother when we were kids and then to his grandchildren. 2019 was the last time he did. He died of

in April.
We read it tonight in his honor. 😢💔

💡 I've recently discovered this academic life hack for presentations:
1⃣Create a *master* slide deck for each area of content expertise
2⃣Add or replace slides in this master deck after creating a new presentation.
No more hunting for that perfect slide!
I'm not surprised that our pts do their homework on us before meeting (Google, etc).
Met a new pt and his 1st priority was expressing sympathy about my dad's death to COVID. I wrote publicly about it and
