In expert centers, the Ross is equally safe but provides better late outcomes than prosthetic AVR. Likewise, TAVR in 65+ as a class I LOE A based on 1 year data in patients >70y is also a misalignment.

Glad to report this multicenter study looking at the incidence of pulmonary homograft dysfunction after the Ross procedure. In this cohort of >450 patients: Only predictor of homograft dysfunction -> Age<45 Risk of dysfunction -> greater in first 18 months and <1%/yr afterwards.

2/ Importantly, 30-day mortality after different valve-related complications varied significantly. They are not all created equal: - Endocarditis: 13.5% - Stroke: 5.6% - Major bleeding: 2.6% - Reoperation: 1.1% Bottom line: No option is perfect. You have to pick your poison.

@DrNivAd Long-term outcomes are excellent, but intricately related to surgical technique. Like any technical operations, it needs dedication and commitment. Think valve-sparing, minimally invasive, robotic etc.. Importantly, it is both teachable and learnable for aortic root surgeons.

There are many ways to avoid PPM (stentless roots, root enlargement etc) and these should always be done at the time of surgery. Importantly, preop echo can predict the risk of PPM (annulus size), so this should be pre-planned. No excuses.

They turn the simplest, safest and most effective op in the book (SAVR) into a less effective one: more pacemakers, PVL, gradients, SVD. All this to save 15 min. Their only potential use is in a redo with a frozen root in an old pt.

It has unique biologic and hemodynamic features, which can only be matched by a living valve, ie, AV repair or #Ross . As surgeons, we need to think long-term for our young pts.

This is an interesting study that underscores the importance of taking time yo know the patient. Assessing an aorta is far more than just a diameter because, as shown, not all dilated aortas are equal. Some are more normal than others!
