Saw a 30 year old woman today… issue…palpitations! she had sought multiple cardiology consultations over the last 6 months with various tests … Hb has been 7-8 all this time. If there is no blood what will the heart pump ! Ladies… take your Hb seriously!

Isolated thrombocytopenia in older patients can be so much other than ITP… just in the last 2 weeks 1. CMML 2. MDS with monosomy 7 3. Marginal Zone lymphoma 4. Autoimmune Myelofibrosis Steroids without marrow exam in 50 plus should really be avoided IMHO…

The simple pleasures of a Hematologist’s life.... no matter how many fancy things you do, a brisk platelet response to IVIG in ITP will always make your day...platelets going from 2,000 to 100,000 in 12 hours........ it’s like magic.... never gets old for me....

We get ADAMTS13 in 2 days in India…I don’t think doctors or patients appreciate that the TAT for majority investigations in India is possibly the fastest in the world…practically 7 day work week, supremely optimised logistics…we love our lab colleagues for their hard work !

These hospitals are getting the ADAMTS13 result one week later by sending the sample elsewhere in the US. They could install exactly the same equipment in their hospital and get the result in 30 minutes. By not doing so, they are plasma exchanging the patient daily for 1 week!1/2

This particular wave of COVID is a different beast... all the old rules may not apply exactly.. seems like patients on a downward spiral need things like ? Pulse steroids? Etoposide? IVIG... more like sec.. HLH... vaccine is fine but better treatment options are badly needed..

We need some kind of national vitamin B12 fortification program… current day Indian diet is extremely poor and the deficiency is a silent epidemic … anemia, impaired cognition , growth and development impairment… something similar to iodized salt programs is required..

If originally the gap between 2doses had been 12weeks, many more people would have had one dose only at the peak of this wave and done much worse...If you are dead before your second dose how does it matter what antibody levels you mount ? 2 doses 4 weeks apart saved more lives .

That being said, widespread effective Teleconsults and easy systems of drug availability and home delivery in India have definitely avoided thousands of hospitalizations. Almost every home has a pulse oximeter ! Wassap and your neighborhood pharmacist to the rescue !

Seen so many women with iron deficiency related symptoms ( headaches, irritability, insomnia) taking benzos and other psychotropic drugs( pacitane, alprax unfortunately too easily available OTC in India).. they can sleep just fine once their ferritin is not in single digits...

Ironically in India lenalidomide and plerixafor have been generic for several years now… lenalidomide is very cheap…RVD is a completely outpatient regimen …… we have had very few reasons to give CTD/ VCD etc…. The lenalidomide issue is a Europe and USA specific problem…

Fruit juices lack fibres, as compared to whole fruits. One tends to overconsume juices as satiety is delayed. Moreover, in packed juices, vitamins may be lesser, as they degenerate over time. Best option is to have whole fruits and the second best is to have fresh fruit juices.

@WesPegden @VincentRK Our hospital is now non- COVID from having 150-200 COVID patients in September... from 50% test +ve rate at peak in our lab we now see one +ve every 3/4 days... total >10,000 samples have been processed just in our lab..one lab in one district with 2-3 other testing facilities..
