Problem: Applicant goes partially matched into preliminary medicine but not radiology. Matches into radiology the following cycle. The IRP for medicine is 3, so the last 2 years of radiology residency are only funded at 50%. The hospital is picking up the bill for the rest.

@mattyfred_ 1 minute from the hospital! I live next door to mine and on electives etc I go home in between patients. On a slow days I get sent home bc the seniors know they can call me and I can come right back if need be.

The number of years Medicare will pay a hospital to train 1 resident is based on the resident’s IRP (initial residency period). IRP is determined by the first specialty a resident matches into and does not change if a resident changes specialties.

@ShoaibRao99 As you mentioned, your funding has been used up. There are programs that choose to take on more residents than they are funded for because they are willing to foot the bill for the increased workforce. Look for programs they have more residents than they are accredited for.

The IRP is the minimum length of time required to become board eligible in that specialty. IM = 3y, Surg = 5y. TY is special and also 5 years. If prelim + advanced are matched at the same time the IRP is based on the advanced specialty. Radiology = 5 which is the max IRP for any.

@alaskanmike2 I emailed some PDs to ask if they considered DO applicants because I noticed they didn’t have any. The few that replied told me they did and had ranked DOs highly in the past. I’ve also heard from residents at other programs that their program will not rank DOs at the top.
