Face masks again. You will still be asked to wear one in a GP practice (and any healthcare setting). By definition there are people there that we need to protect. Please do not be surprised, annoyed or angry that this is the case, particularly with our staff who are just helping.

GP ALERT. 1/2 There will be significant delays and difficulties today in all practices. At midnight last night there were nearly 400 calls in Belfast alone to be returned (by 2 GPs on the red eye). Huge and unsafe pressure for those involved.5

Never ever forget that patients donโ€™t โ€˜waitโ€™ on waiting lists. They get older, frailer, sorer, sicker and very frequently poorer by having to pay. We see them many, many, many times in GP

From the recent NI hospital waiting list stats, 1 in 20 people in waiting lists in NI have been waiting more than *5 years and 13 weeks*. In England waiting lists over 2 years have been nearly eliminated - fewer than 1 in 24,000 on waiting lists in England waited over 2 years.

Can I please just correct some reporting today. GPโ€™s will NOT be withdrawing any services. We will be working with HSCB and Dept to ensure an effective supply of PPE to practices and this has already improved over the last week. We have worked very hard to ensure safe practice.

It is so difficult. I had 2 patients today, both waiting over 3 years, asking to go privately and clearly canโ€™t afford it. Most frustrating part is that they and I know exactly what they need but have to wait endlessly for an appt just to confirm. We need a complete redesign.

Very important and to stress do NOT contact your GP for letter. We do not want to get into conflict with our patients (vast majority of whom should wear a mask), and do not want to become complicit in spread of infection by providing any tenuous exemption.

An alert for the week ahead. We are getting multiple reports of many practice staff and GPs testing positive. They will isolate as per current guidelines but it will leave many practices short staffed with access compromised. Urgent cases will be prioritised.

There is also evidence that a passport increases vaccination rates (a good thing), and particularly from France and from Ireland and more recently Scotland There is also evidence that they reduce cases, although they are often introduced with other measures hence combined effect

Passports or vaccines do not produce infection free environments, this needs complimentary measures like lat flow tests, masks and social distancing. If everyone is vaccinated though it minimises the effect of a positive case, or a false -ve lft

@timmckane Exceptionally few exemptions, mainly a physical deformity meaning you canโ€™t attach it, significant respiratory illness with a documented drop in oxygen sats, or a significant mental illness. All in all, the vast majority of people can and should be wearing them.

GP cost pressures. Practices are small businesses and simple ones at that, usually with one contract with NHS. We canโ€™t put prices up, the only way to meet costs is reduced spend, often on staff and on GPs. This will inevitably harm access further when we need to increase it.

General Practice update. All practices are open and seeing patients and have been throughout. Demand is at nearly 200k contacts per week with 1/3 of those face to face. It is phenomenally busy and was struggling pre covid and even more so now.
