Today BCBS wouldn’t approve surgery for acute ACL tear in young female athlete. They now are using a third party company called @CarelonHealth to decide if my patients board certified fellowship trained former NFL ortho surgeon recommended the correct care. This has to stop.

Had a patient of mine hurt her knee in London while at school. Wouldn’t Xray her,35weeks minimum wait to see an Ortho, no MRI avail .Would only allow her to do physio? Flew home to see me. Anyone who thinks NHS is a model to follow in US,is mistaken.

Hey @Cigna You denied my patients shoulder MRI,she had to pay cash to obtain this which showed a SLAP rotator cuff injury after trauma which also fx her dist radius. You now say she must do 3mos of PT and r denying surgery.This is practicing medicine and not legal in TX.

Just stop it. If you want to practice medicine,take the MCAT, apply to med school and stop trying to legislate something your not. Never thought in my professional career this would even be an issue. Nurses are not doctors and can not practice medicine.

Can we agree that NPs are a very valuable part of the healthcare team but are not capable of replacing physicians. If someone wants to practice medicine, they must go to med school. We shouldn’t allow NPs to practice without oversight. Not sure why this is even a question.

Another week of UHC denying claims and surgery.Patient with very large meniscus tear. Pt can’t walk without limp,do daily activities but UHC it’s “not that bad”. We need legislation banning docs working for INS and remove any sort of pre-certs. INS industry needs a reset.

6 yrs ago a financial planner convinced me I should get a whole life policy as investment and addl life insurance cost 50k was told would go up in value. Person selling it made about 13,500 off the top today the value is 15k. Don’t fall for this whole life scam. @olsonplanner

In 1990 srgns under Medicare were paid avg $3,258 for a knee replacement today avg $1098. Considering inflation with no increase surgeons should be paid $7,656 today yet our expenses have gone up 2.35x,hospitals cont to receive ⬆️ reimbursement.

PBMs charged Medicaid (taxpayers) $147 for a heartburn med. PBM only paid the pharmacy $17 & pocketed $130 for themselves as payment processors. Imagine buying milk & paying the credit card co $130 for processing. Only corruption would allow this to continue. #StopPBMabuse

Have a 12y patient with patella fx with chondral injury. Surgery not approved/cx because BCBS says must see PCP first and then refer to ortho. No wonder PCPs r burning out this is complete waste of resources and abuse of our primary colleagues leading to delay in care.

@kksheld @SecretaryCarson In 1990 I was living in a 5th wheel trailer newly married, sacking groceries and going to a junior college. My mother in law gave me a copy of the book Gifted Hands. No one in my family had gone to college. That book and Dr Ben Carson inspired me to become an orthopedic srgn.

No reason to ban Physician Owned hospitals. Lawmakers gave the keys to the hen house to the foxes. The hypocrisy is amazing. Hospitals and now HC companies like CVS can literally own everything and employ docs,NPs.Ok to have corps own,just not doctors.

31 years ago yesterday I celebrated marrying my best friend. We were 18 and 19yo. Undergrad, 2 grad programs, med school, residency, fellowship, practice,3 kids and now 6 grandkids we made it. Thankful!

Why do we allow hospitals to employ doctors by pseudo physician groups of which they fund and support? Who are we kidding, this is corporate practice of medicine and hurts patients by driving up HC costs and harms Pvt Practice. Where are the med associations? Prove me wrong.

@benshapiro I have two doctorate degrees, but you can call me Robert. If a flight attendant on a plane asks “Is their a doctor on board”, u know what she’s asking. Most of my medical colleagues ask to be called by first name. What u choose to call me doesn’t change who I am.

6 weeks ago I told you our friend underwent a heart transplant, last night we had a “New Heart” celebration for him. He looked totally normal and is back exercising. EF from 30 to over 60. His wife and son got their husband/father back I can’t believe we can do this.

50y male I repaired Achilles 10y ago. Uneventful recovery returned to full activities:running,hiking etc. Presents to clinic 3days after pain and swelling with no fever. X-rays show soft tissue swelling. Aspirated thick white toothpaste material.What’s your Ddx? #orthotwitter

@AshleyGWinter I know Ashley when it’s your own, for a brief second we loose our objectivity. I have 3 grown children, 7 grandchildren and when these spontaneous uncontrolled outbursts occur sometimes I think, we should maybe call a doctor😃 BTW this doesn’t change as they age,always our

Saw a patient with comminuted closed DR fracture,needs surg. Pt has a plastic card they carry so thinks they have access to care. Ded over 1Ok Fam ded 17k. They now need surgery and guess what no facility or surgeon is “in network”. Good job UHC/Optum selling this. #orthotwitter

So since @generalorthomd shared his peer to peer today, I’ll share mine. Had a physical therapist review my patients MACI surgery and denied on behalf of BCBS. A PT,not a doctor or Ortho surgeon. The denial letter was nonsensical as u would expect from non Ortho.

A black box in the OR still won’t get my room turned over any faster and I would venture most surgeons/OR nurses wouldn’t want to work in one with it. This is what happens when doctors turn over running hospitals to corporations: “How can we extract more 💰while paying staff

‘OR black box’ *sounds* like a good idea But it’s just a way for some company to milk hospital $$$ while providers get paid less and costs soar and we get no real benefit Gonna raise a monster round though 😁

@JoeySalads This is my brother in law in CA. He is Board Certified Emergency Medicine physician and runs one of largest urgent care groups. He simply reported on his own data.
