I would slip out of the back doors of my medical school into the back doors of our cancer hospital for my after hours infusions. I would always carry the ID bracelet, so I wouldn't forget to take it off. I was thin. It was seen as fashionable.

It will always be the (white) public health nutrition people. Like my sworn enemies since MPH times. An apple a day does not keep the doctor away when access to both apples and doctors are shaped by structural racism.

It was hard for me to hide in plain sight. But I wanted to be treated like a full human being, and I saw how my profession treats people with cancer. Black people with cancer. How the world does. And, he didn't owe us a whisper about his illness.

I'm not good with the way that clinicians are posting about patients that they work with on their social media. I'm sure there are very specific situations when it's ethically OK. I usually find it deeply troubling. I don't want to recognize myself on my clinician's social media.

When I was a medical student I eventually just had to say "I'm going to leave now" because after trying the "anything else I can help with" and even the "when is a good time for me to end for the day" I would be there until 9pm with an expectation to be back at 3:30am. MS3.

Advice for the next time I guess? When someone calls you a "hero," or a group of people "heroes," it's usually a setup for profound, intentional neglect. Heroes aren't human. They don't need care, understanding, safety, meal breaks. They don't have boundaries. They can be used.

I cannot be the Black doctor for every Black patient. And the reason that a Black patient survives should not be because they have a Black doctor. And there should be many, many more Black doctors simply as an end in and of itself. It isn't utilitarian, some "diversity" goal.

And this, in an of itself, doesn't make me an exceptional doctor. It makes me a Black doctor in an exceptionally anti-Black system. I resent it deeply every time. Particularly the fact that there is really so little that I can do.

This is a terrible, harmful take. And I'm sure it is unfortunately quite common among physicians. Patients (aka people) should be able to determine how and when they wish to receive information that is *about them*. Paternalism must go.

Imagine finding out your unborn baby had a terminal chromosomal abnormality. At 11pm. On your iPhone. The CURES Act is harmful for patients. Not every piece of medical information should be immediately available. It further erodes down the strained physician-patient relationship.

I'm not sure who needs this, but it's almost always better to say "Black people." Or "Black physicians." Or "Black patients." Instead of taking the word "Black" and pluralizing it. "Black" isn't the noun you may think it is and person-first language is just way better. 👍🏾

I want Black mistakes and second chances and failures. I want to be able to be as human as I see my colleagues being. Late for a morning conference without it impugning your character. A typo in an email without it meaning you lack the ability to succeed.

This means you have much work to do. That work is not #DEI work. It is not "implicit" bias training. It is not organizing lectures on "diversity." It is not "disparities" research. It is not putting a #BLM pin on your coat or hashtag in your bio, radical though this may feel.

It's amazing how medical folks continue to miss what is *actually* important to patients. Transport is important. Parking cost is important. This is financial and logistical suffering (pall care folks understand this) and it can often predominate. Thank you Dr. @fumikochino 👏🏾♥️

My father was a #USPS mail carrier for his entire career. It was a functional, affordable, excellent system. He was never paid much but it was a stable job. We had health insurance that allowed our family not to go bankrupt with my childhood health conditions.

My heart is heavy thinking abt Dr. Preston Phillips who was murdered in the recent Tulsa mass shooting. Fewer than 2% of orthopedic surgeons are Black. He graduated from @harvardmed & trained at @BIDMChealth . Just thinking abt how many hurdles he had to overcome in his life.

Progression-free survival...I get it. I also deeply regret that we use it so much. It means something. But it doesn't mean what we use it to mean colloquially, which is freedom from your cancer bothering you. That's what I want as a patient. Freedom from bother from cancer.

It's wild how many times I'm explaining Mark Cubans Cost Plus Pharmacy in clinic visits. It feels like I'm in an alternate universe, like okay CVS and your health insurance have decided 30 pills is 2k and out of pocket at CVS it's 1k but at Mark Cuban's place it's 5.30.

🙌🏾 Love your patients. Love your colleagues. Love your (chosen) family. Love yourself. The institution can be a vehicle. But on that day when you can no longer work...they will replace you. You are irreplaceable to the individuals who you have loved and who have loved you back.

When I tell this story to Black mentees it is a lightbulb moment for many. We are not often allowed to fail. Even in minor ways. And if we do there aren't often second chances. Forget Black Excellence. We are excellent in the academy, we had to be to even get a foot in the door.

I mean every time I talk about the need to improve access to cancer treatment for excluded people, academics want to come with "what about prevention" arguments. What about it? We can't pit prevention and treatment at odds, we have to imagine more for all of us.
