Playing around with some stuff… This is really hard! My tib post was screaming at me 💪🏼🤣 Tib post has short muscle fibres - it’s not a primary mover, so exercises like this challenge it in a more functional way than something like banded inversion. Progress with a heel raise🦶🏼

Midfoot Pain 🦶🏼👈🏼 Excluding neural, so remember to consider superficial peroneal nerve for dorsal foot & sural nerve for lateral. Deep peroneal is 1-2 webspace (more forefoot). BSI of navicular, base of 2nd MT & Jones # (zone 2 5th MT) are highest risk areas ⚠️

Sesamoiditis or FHB tendinopathy? 🤔 Mini thread & case study 🧵 1/5 The sesamoids live within the FHB’s distal tendons & articulate with the head of the 1st metatarsal, acting as a fulcrum to increase the leverage of FHB/FHL. But is all ‘sesamoid’ pain actually the sesamoids?

Something I picked up from @Rob_NBF I now use this all the time with my patients… The 6 basic movements needed to train every major muscle group in the body. So simple & efficient: 1 Squat 2 Hinge 3 Vertical push 4 Vertical pull 5 Horizontal push 6 Horizontal pull

In my recent Achilles’ tendinopathy post, I used the terms paratenonitis, tenosynovitis & sheath inflammation interchangeably. While I was trying to describe the pathology in order to explain how the rehab differed, it was inaccurate! Great paper here 👇🏼

I am officially gallstone & gallbladderless! 🙌🏼 The anaesthetist said it was the biggest gallbladder he’s ever seen, but they managed to do the operation laparoscopically, so I’ll be back to doing crunches in no time (a few weeks) 😉 Extremely relieved it’s all over 🙏🏻

4 wks ago today I had my cholescestectomy. The 2nd - 4th days were the hardest! Wish I could have a ‘control’ me who sat on the sofa for a month to see how we compared at this point. Feeling strong & ready to start back at work next wk. Might send Antonio Conte my rehab plan 😉💪🏼

Exercises for turnout - Reverse clam It's almost impossible to cheat, so long as you keep the working leg just behind neutral. Pop a hand on your bum to feel your external rotators engaging. Concentrate on the movement coming from the posterior hip 🩰

Sinus tarsi syndrome = outdated!🦶🏼 We know that most, if not all ST pain has a clear etiology; planovalgus foot type = lateral impingement, subtalar instability +/- lateral ligament rupture, intra-articular ST pathology. Recent paper here with diagnostic & treatment algorithm 📄

Taping for posterior ankle impingement PAI (in contrast to ALI) often presents alongside ongoing ankle instability, so this taping includes a heel-lock which helps support the subtalar joint... Use rigid tape over the anterior ankle to further restrict ROM ⚽️🤸🏿‍♀️🏏🩰🏈⛹🏿🧗🏽

Had a bit of a shocker this week to find out a 20 yr history of stomach pain & more recent palpable lump in my right side was this enormous gall bladder + stone. A great example of a missed diagnosis. I’m shell shocked… I’d convinced myself my pain was psychosomatic 🤦🏼‍♀️😔

When manual therapy is worth doing: 40 yr old dancer who had stopped performing & was devastated. Hallux rigidus with dorsal osteophytes. Unable to rise or jump. 5 mins massage to MLA, traction to joint & lateral glides to toe = full rise height 🦶🏼 Tears of joy ☺️ Caveats below👇🏼

New foot pathology learnt this week🦶🏼 Cuboid pulley lesion: Usually asymptomatic bone stress response on MRI where the peroneus longus tendon exerts force across the cuboid as it wraps around it. Associated with PL tenosynovitis & inflammatory arthritis 🔗

Mid-stage ankle rehab 🦶🏼 Progressions: 🔸Add weighted objects (the 5kg dumbbell threw me off balance)! 🔸Move objects further away (the ball behind me was REALLY tricky) 🔸Raise the heel on a yoga block/towel 🔸Add more objects and do a timed version - most items in 20 seconds ⏳

Looking for a way to Ax intrinsic foot strength? Try the Paper Grip Test for the 1st MTPJ. It works like Froment’s Sign for ulnar nerve palsy in the hand. (The UN innervates the intrinsic hand muscles). A +ve sign is the same in the foot = unable to grip or clawing #Intrinsics 🦶🏼

Isometrics for patella tendinopathy in dancer who had surgery & 3 x CSI into tendon 7 yrs ago. Only 9 days later he’s noticing a massive improvement in pain/function. Not advised to load previously. Gone from 27kgs to 73kgs in a week & ready to progress again 💪🏼

Research consistently shows that dancers will perform in pain, are reluctant to report injuries, & feel like pain as high as 6/10 VAS is ‘normal’. So when a dancer finds the time & money to see you about an injury, it’s significant. Listen to them… 🙏🏻 Links in next tweet 🔗 1/2

33 M R occipital triangle; tight & rope-like with L side flexion. 3/12 Hx with no MOI. Worse with R shoulder elevation. Nil neuro/red flags. No pain O/P anywhere! No improvement in 3/52 with soft tissue work. Scalenes? Not even sure it’s myogenic at this point 🤯 Any ideas? 🤔🙏🏻

🚨IT’S NEVER THE HIP FLEXOR 🚨 @DrJN_SportsMed Unless your patient describes overload after doing activities like these, consider other differentials. Important to rule out stress fractures or hip joint pathology. The HF may be part of the picture, but it’s often not the driver.

Heel Lifts. So useful for so many foot and ankle injuries: Achilles tendinopathy (mid-portion & insertional), paratenonitis, anterior impingement, plantar fasciopathy, peroneal tendinopathy, calf strains… NB: These form part of management only & won’t be suitable for all Pts ☝🏼

Proximal hamstring tendinopathy is different in people who are very flexible - dancers, gymnasts etc. They're often injured in end-range (extreme outer range) positions, so you need to include these when they get to the appropriate place in their rehab journey 🤸🏻 Thread 🧵 1/4

Lat ankle sprain 3 days & 4 wks after injury. PF + IV mechanism landing on lateral foot out of a jump - high energy. X-ray cleared by A&E but I suspect there’s a talar dome injury. Stiff T/C joint, difficulty walking, tender talar dome, muscle guarding, AM stiffness. MRI needed🦶🏼

I love anatomy. Currently experiencing pretty awful shoulder pain from all the air they pumped into my abdomen for the cholescestectomy! It’s absolutely textbook - just like the pic 🤩 Also extremely painful. I’m trying to focus on the positives… 🤣 #CliniciansWithPain
