1/ Social distancing seems to be working in SF, but to what end? There doesn’t seem to be any good data that the virus is going to die out come summer. Looking at @goforward ‘s test data, we’re only seeing ~20% test positive so presume herd immunity isn’t likely any time soon.

22/ Big question: does telemed simply replace in-person visits (good for pts & saves a few bucks on space/staff... but not transformative). To be transformative, need telemed coupled w/ home monitoring/management, which may prevent exacerbations, ED visits, & hospitalizations.

2/ Seems that unless you believe a vaccine or treatment is going to be available in a reasonable timeframe, we’re not going to be out of our homes any time soon. The economic impact is disproprtionately affecting lower income folks, so we need to take that impact seriously.

3/ Vaccine development, trials, and distribution are likely 1yr+ away. Treatments are developing quickly (e.g., hydroxychloroquine + z-pak) but thus far not proven to be hugely material for outcomes (though still early). We’re setting up more hospital beds but not materially.

We're seeing huge uptick in in-box messages for MDs during Covid – now seems like biggest driver of MD burnout. The fundamental problem: we turned on 24/7/365 access for patients (who of course like it) with no operational or business model to handle it. Crucial that we fix this.
