Thanks to Dr. Kelsey Lock for teaching us about the efficacy of amiodarone v lidocaine for VT/VF related IHCA. Lidocaine was shown to have higher rates of ROSC, 24h survival, survival to DC, and favorable neuro outcomes. So are you #teamamidoarone or #teamlidocaine ?

This systematic review included 30 studies with varied settings. Overall, pharmacist involvement is beneficial for initiating time critical meds, ⬆️med appropriateness, and ⬆️guideline adherence. It was underpowered to ID patient oriented outcomes like mortality or LOS. #EMRx

Thanks to everyone who joined us today. Dr. Apato ( @amanda_apato18 ) shared with us about the use of SQ insulin in mild-mod DKA. The SQuID protocol used 0.2 units/kg lispro every 2 hours. Overall, there was no difference in safety events &shorted ED LOS for SQ patients. @RushPharm

AHA 2020: IV preferred over IO in cardiac arrest. Recent meta-analyses including 5 retrospective studies found improved ROSC & survival to hospital discharge. However, no differences were found in subgroup analyses of 2 prospective RCTs. See here:

The TICH-NOAC trial provided 1g TXA to patients with non-traumatic ICH within 12 hours from onset and who had their last NOAC dose within 48 hours. TXA did not provide a statistically significant difference in hematoma expansion. Rates of death and disability were similar.

So when should patients get NAC? Indications for NAC include: acute toxic level on the Rumack-Matthew nomogram, APAP level > 10 mg/mL with unknown ingestion time, acute doses of 150 mg/kg or 10 g in adults, chronic ingestions > 4g/day with elevated LFTs.

Thanks to Dr. Doug Nulph for his discussion about SVT and adenosine dosing. Pts who had successful conversion with 6 mg were significantly lower in weight compared to who failed conversion. Should we be starting with 12 mg in our larger patients? Considering alternative agents?

The PROCOAG trial included trauma patients at risk for massive transfusion. Overall, the use of 25 units/kg 4FPCC did not improve outcomes including number of blood products required, mortality, or ICU free days. A potential increased risk for VTE was seen. #EMRx #TwitterRx

Here is a diagram which delineates the mechanism of NAC in acetaminophen toxicity. When glutathione store are low, NAPQI can accumulate and lead to direct damage to the liver. NAC serves as a glutathione donor to allow for the removal of NAPQI as cysteine and mercapturic acid.
