Medical students notoriously have a hard time entering the OR for the 1st time. Now imagine the added weight Muslim women carry in figuring out hijab-friendly sterile dress options. PPE and scrubs are designed for men's bodies - large sizes, long pants, low-cut tops. 1/

Hear me out- we technically don't need ERAS. The Match is run by NRMP using AAMC IDs, which we get from applying to med school. Disruption: make a new platform for the application review, use AAMC IDs, and still run the match algorithm.

- hanging out near nursing station around shift changes because fresh eyes -> new questions/concerns

Spending the year in a predominantly lipids focused lab doesn’t bring a ton of overlap with Oto but today I learned about pathognomonic bright orange tonsils (cholesterol deposition) in Tangier Disease from a labmate! Deficient ABCA1, low HDL, hepatosplenomegaly, CAD. *very* rare

5. Similarly, never ever tell a patient they NEED a surgery/procedure unless you’re the one doing it. Surgeons are not technicians - they will evaluate the patient and provide their plan, which may or may not include the surgery you referred the patient for.
I was already getting teary-eyed watching this and then saw my own picture from AAO 2019 pop up -- my jaw hit the floor! So excited to 🤞hopefully 🤞 join a field with so many incredible trailblazers ahead -- and mentor the next generation.
4. Never ever ever tell someone they’re having surgery/a procedure unless you’ve heard it DIRECTLY from THE PERFORMING service. Ex: if GI said gen surg was gonna take a pt to the OR, check w gen surg. Do not tell the patient/fam they’re going to the OR.
Subtle joys of match day for non-applicants: watching the parents in envelope-opening videos (my heart melts for immigrant parents especially), getting texts from mentees/being a part of their journey, reflecting on your growth since your match day ✨

I also didn't really believe in the mystical lymphatics until seeing a chyle leak in the OR. Greasy fluid slowly oozing out in the neck = impossible to forget that certain fats go straight from gut thru lymphatics (chylomicrons, appropriately). + drainage and immune functions.
2. YOU are “OSH” to everyone else…treat (and document) the way you’d want to be discussed. Check your assumptions and lead your chart digging with genuine curiosity and openness to being wrong.

6. Nothing is magically foolproof or done better because it’s done at a top academic institute. The human body doesn’t change based on which hospital it went to; an artery is an artery anywhere. Recognize the crutches you have in a well-resources hospital.
highlight of my week has been connecting a new M1 with other students and faculty who do the things she’s interested in. 10/10 recommend for fellow M4s. Being on the tail end means we know our schools’ landscape and should help new students build community in a virtual world
9. Don’t take anything personally. If someone is acting weird towards you, 1) reflect on if you could have done something that made their job harder or easier 2) recognize that most tantrums come from a place of insecurity

What message does it send when women, of all backgrounds, quite literally don't "fit" in the profession? Especially now, misfitting PPE can be a matter of life or death. While in the general population, COVID impacts men more, in HCW this is flipped.
16. Load the boat. Don’t be a cowboy. Ask for help early and often. Your staff and chief likely do not know what you know about a patient because *you* got the first page- make an assessment, but don’t sit on it.
11. If you’re having a tough conversation, be mindful of your surroundings. If you’re telling a funny story, be mindful of your surroundings.
12. If you’re putting a lot of force on an instrument or having to Jerry rig it, consider that you’re using it wrong.

Reflecting on this poem and call to action by Persian poet Saadi Shirazi in this new year. Written in 1258 AD and carried into the United Nations' Universal Declaration of Human Rights almost 700 years later in 1948.
Women have noted masks and other PPE are too large to create protective barriers or seals. This is a failure in design. When designers are not well-versed in inclusivity, they fail to meet a basic principle of universal design: the product has to work for everyone who uses it.
So my first (short) day is done, at least in the hospital. Here are my observations:
1. I have a small head. No, this is not news, but try to put a bunch of PPE on a small head, and you’ll see why this matters (see video)
2. Things that were not normal now seem normal.
